๐Ÿ”ฌ Pharmacology (PK/PD)

Pharmacokinetics

AbsorptionWell absorbed orally, approximately 80-90% absorbed from the gastrointestinal tract.
DistributionWidely distributed into body tissues and fluids, including cerebrospinal fluid (CSF) with levels about 60-80% of plasma.
Protein BindingLow, approximately 2-4%.
MetabolismPartially metabolized in the liver to 5-fluorouracil (5-FU) by gut bacteria and possibly by host cells.
Half-Life3-6 hours (normal renal function); prolonged in renal impairment.
ExcretionPrimarily excreted unchanged in urine (80-90%) via glomerular filtration.
BioavailabilityApproximately 80-90%.
Onset of ActionVariable; antifungal effect may be seen within 24-48 hours.
Peak Plasma Time1-2 hours after oral administration.
Duration of ActionDosing interval typically every 6 hours due to short half-life.

How It Works

Flucytosine is converted intracellularly to 5-fluorouracil, which inhibits fungal RNA and DNA synthesis by incorporating into RNA and inhibiting thymidylate synthase.

Mechanism Steps

1Flucytosine enters fungal cells via cytosine permease.
2Converted to 5-fluorouracil (5-FU) by cytosine deaminase.
35-FU is further metabolized to 5-fluorouridine triphosphate (5-FUTP) and 5-fluoro-2'-deoxyuridine-5'-monophosphate (5-FdUMP).
