Phenytoin is contraindicated in patients with hypersensitivity to hydantoins, those with sinus bradycardia, sinoatrial block, second- or third-degree AV block, or Adams-Stokes syndrome. It should not be used in patients with a history of liver disease or porphyria. Also, avoid in pregnancy (category D) due to risk of fetal harm.

Phenytoin can cause serious skin reactions like Stevens-Johnson syndrome, especially in patients of Asian descent with a specific gene (HLA-B*1502). It may also cause liver damage, so regular liver function tests are recommended. Patients should be monitored for signs of toxicity such as nystagmus, ataxia, and slurred speech. Do not stop the medication abruptly as it may lead to withdrawal seizures.

In case of overdose, seek immediate medical attention. Symptoms may include nystagmus, ataxia, dysarthria, lethargy, and coma. Treatment is supportive and may include gastric lavage and activated charcoal. Hemodialysis may be effective in severe cases.
