1. Clinical Overview

Sodium Chloride 0.65% w/v is a sterile, isotonic, buffered, or non-buffered aqueous solution for nasal irrigation and ocular use. It is classified as an electrolyte replenisher and nasal decongestant. In the Indian context, it is widely used as a non-pharmacological, physiological saline solution for managing symptoms of allergic rhinitis, sinusitis, and dry eye syndrome. Its 0.65% concentration is slightly hypotonic compared to plasma but is well-tolerated for mucosal irrigation.

2. Mechanism of Action

As a slightly hypotonic saline solution, it works by moistening the nasal and ocular mucosal surfaces. It helps to liquefy viscous mucus, facilitating its clearance (mucociliary clearance). It washes away allergens, irritants, and crusts, providing a soothing effect. It helps restore the physiological ionic environment on the mucosal surface.

3. Indications & Uses

  • Symptomatic relief in Allergic Rhinitis (Hay Fever)
  • Symptomatic relief in Non-Allergic (Vasomotor) Rhinitis
  • Adjuvant therapy in Acute & Chronic Sinusitis
  • Dry Eye Syndrome (as an ocular lubricant wash)
  • Nasal dryness due to air conditioning, heating, or dry climates

4. Dosage & Administration

Adult Dosage: Nasal Spray/Drops: 2-3 sprays or drops into each nostril, 3-6 times daily or as needed. Nasal Irrigation (via bulb syringe/neti pot): 100-250 ml per nostril, 1-2 times daily.

Administration: For Nasal Spray: Blow nose gently. Shake bottle. Tilt head slightly forward. Insert tip into nostril, spray away from nasal septum. Breathe gently inward. Repeat for other nostril. For Nasal Irrigation: Use sterile, lukewarm solution. Use proper device (neti pot, bulb). Lean over sink, head tilted. Pour solution into one nostril, allowing it to drain from the other. Blow nose gently afterward. For Eyes: Tilt head back, pull lower eyelid down, instill 1-2 drops. Close eye gently for 30 seconds.

5. Side Effects

Common side effects may include:

  • Mild, transient nasal burning or stinging
  • Sneezing immediately after use
  • Mild bitter taste in mouth (nasal use)
  • Watery eyes

7. Patient Counselling

  • DO use sterile water or previously boiled (and cooled) water for nasal irrigation.
  • DO clean the irrigation device (neti pot, bulb) thoroughly after each use and allow to air dry.
  • DO blow your nose gently before using nasal spray.
  • DO NOT share nasal spray bottles or droppers with others.
  • DO NOT use if the solution is cloudy or contains particles.
  • DO NOT use for injection.

8. Toxicology & Storage

Overdose: Topical: Local irritation, edema. Systemic (from ingestion or inappropriate IV use): Hypernatremia, hypertension, pulmonary edema, thirst, restlessness, seizures.

Storage: Store at room temperature (15-30°C). Protect from direct sunlight and excessive heat. Keep the container tightly closed when not in use to prevent contamination. Do not freeze. Keep out of reach of children. Discard any unused solution as per package instructions (typically 28 days after opening for preservative-free multi-dose vials).
