1. Clinical Overview

Silymarin is a standardized extract from the seeds of the milk thistle plant (Silybum marianum). It is a complex of flavonolignans, primarily consisting of silybin (silibinin), silychristin, and silydianin. In the Indian context, it is widely used as a hepatoprotective agent for various liver disorders, including toxic, metabolic, and drug-induced liver injuries. It exhibits antioxidant, anti-inflammatory, antifibrotic, and membrane-stabilizing properties.

2. Mechanism of Action

Silymarin's hepatoprotective action is multifactorial: 1) It acts as a direct antioxidant by scavenging free radicals. 2) It increases the intracellular concentration of glutathione, a key cellular antioxidant. 3) It stabilizes hepatocyte membranes by inhibiting the binding of toxins (like amatoxin from death cap mushroom) to membrane receptors. 4) It stimulates ribosomal RNA polymerase, promoting hepatocyte protein synthesis and regeneration. 5) It exerts anti-inflammatory effects by inhibiting NF-κB translocation. 6) It has antifibrotic properties by inhibiting hepatic stellate cell activation.

3. Indications & Uses

  • Supportive treatment in toxic liver damage (e.g., alcohol-induced, drug-induced hepatotoxicity)
  • Chronic inflammatory liver diseases (e.g., chronic hepatitis, non-alcoholic fatty liver disease (NAFLD), non-alcoholic steatohepatitis (NASH))
  • Cirrhosis of the liver as supportive therapy

4. Dosage & Administration

Adult Dosage: 70mg to 140mg three times daily. For 35mg/5ml syrup: 10ml (70mg) to 20ml (140mg) thrice daily, preferably before meals.

Administration: Administer orally. Shake the bottle well before use. Use the measuring cup or spoon provided. Preferably take 30 minutes before meals for optimal absorption. Can be taken with a small amount of food if GI upset occurs.

5. Side Effects

Common side effects may include:

  • Mild laxative effect (due to increased bile secretion)
  • Nausea
  • Epigastric discomfort
  • Allergic skin reactions (pruritus, rash)

7. Patient Counselling

  • Do take the medicine regularly as prescribed for the full duration.
  • Do shake the bottle well before measuring each dose.
  • Do use the provided measuring device for accuracy.
  • Do inform your doctor about all other medicines and supplements you are taking.
  • Don't consume alcohol, as it worsens liver damage and counteracts the medicine's benefit.
  • Don't use it as a substitute for avoiding the cause of liver damage (e.g., alcohol, hepatotoxic drugs).

8. Toxicology & Storage

Overdose: No reports of serious overdose in humans. Based on high safety margin, symptoms may include severe gastrointestinal upset (nausea, diarrhea, abdominal cramps).

Storage: Store below 30°C. Protect from light and moisture. Keep the bottle tightly closed. Keep out of reach of children. Do not freeze. Discard any unused medicine 4 weeks after opening the bottle.
