Mfr: Mankind Pharma Ltd | Form: Syrup

📋 Clinical Overview

Silymarin is a standardized extract from the seeds of the milk thistle plant (Silybum marianum). It is a complex of flavonolignans, primarily consisting of silybin (silibinin), silychristin, and silydianin. In the Indian context, it is widely used as a hepatoprotective agent for various liver disorders, including toxic, metabolic, and drug-induced liver injuries. It exhibits antioxidant, anti-inflammatory, antifibrotic, and membrane-stabilizing properties.

💊 Dosage & Administration

Adult: 70mg to 140mg three times daily. For 35mg/5ml syrup: 10ml (70mg) to 20ml (140mg) thrice daily, preferably before meals.

Note: Administer orally. Shake the bottle well before use. Use the measuring cup or spoon provided. Preferably take 30 minutes before meals for optimal absorption. Can be taken with a small amount of food if GI upset occurs.

⚠ Contraindications

  • Known hypersensitivity to Silymarin, milk thistle, or any component of the formulation (e.g., excipients in syrup)

🔬 Mechanism of Action

Silymarin's hepatoprotective action is multifactorial: 1) It acts as a direct antioxidant by scavenging free radicals. 2) It increases the intracellular concentration of glutathione, a key cellular antioxidant. 3) It stabilizes hepatocyte membranes by inhibiting the binding of toxins (like amatoxin from death cap mushroom) to membrane receptors. 4) It stimulates ribosomal RNA polymerase, promoting hepatocyte protein synthesis and regeneration. 5) It exerts anti-inflammatory effects by inhibiting NF-ÎșB translocation. 6) It has antifibrotic properties by inhibiting hepatic stellate cell activation.

đŸ€• Side Effects

  • Mild laxative effect (due to increased bile secretion)
  • Nausea
  • Epigastric discomfort
  • Allergic skin reactions (pruritus, rash)

đŸ€° Special Populations

Pregnancy: Category C (US FDA). Animal reproduction studies are insufficient. Use only if clearly needed and potential benefit justifies potential risk. Not recommended for routine use.

Driving: No known effects. Unlikely to impair ability to drive or operate machinery.

MetronidazoleSilymarin may increase the elimination of metronidazole, potentially reducing its efficacy.Moderate
Rosuvastatin, Simvastatin, other statinsTheoretical potential for increased statin levels and risk of myopathy due to inhibition of CYP2C9 and OATP1B1 by silymarin. Clinical significance is unclear but monitor for muscle pain.Moderate
Diazepam, Midazolam (CYP3A4 substrates)Silymarin may inhibit CYP3A4, potentially increasing levels and sedation of these drugs.Moderate
Antidiabetic drugs (e.g., Glimepiride, Metformin)Silymarin may potentiate hypoglycemic effect due to possible improvement in insulin resistance. Monitor blood glucose levels.Moderate
Antipsychotics (e.g., Haloperidol) & PhenytoinSilymarin may protect against drug-induced hepatotoxicity caused by these agents.Beneficial
