1. Clinical Overview
A topical nasal decongestant and soothing solution combining two sympathomimetic amines (Naphazoline and Phenylephrine) for rapid vasoconstriction with two counterirritants (Camphor and Menthol) for a cooling sensation and symptomatic relief of nasal congestion. Primarily used for temporary relief from nasal and sinus congestion associated with allergic rhinitis, common cold, and sinusitis. It is intended for short-term use only.
| Onset | Duration | Bioavailability |
|---|---|---|
| Naphazoline: 5-10 minutes; Phenylephrine: 5-10 minutes; Camphor/Menthol: Immediate cooling sensation. | Naphazoline: 4-6 hours; Phenylephrine: 2-4 hours; Camphor/Menthol: 1-2 hours. | Topical nasal application; systemic absorption is minimal (<10%) when used as directed. |
2. Mechanism of Action
Naphazoline and Phenylephrine are direct-acting sympathomimetic amines that stimulate alpha-1 adrenergic receptors on the vascular smooth muscle within the nasal mucosa. This causes vasoconstriction, leading to reduced blood flow, decreased edema, and shrinkage of swollen nasal tissues, thereby relieving nasal obstruction. Camphor and Menthol act as topical counterirritants and mild local anesthetics. They stimulate cold-sensitive TRPM8 receptors in the nasal mucosa and skin, producing a cooling sensation that subjectively improves airflow and provides relief from irritation.
3. Indications & Uses
- Temporary relief of nasal congestion due to common cold
- Temporary relief of nasal congestion due to allergic rhinitis (hay fever)
- Temporary relief of nasal/sinus congestion due to sinusitis
4. Dosage & Administration
Adult Dosage: 2-3 drops or 1-2 sprays into each nostril. Not to be used more than every 4-6 hours. Maximum: 3-4 times in 24 hours.
Administration: 1. Blow nose gently to clear nostrils. 2. Tilt head back slightly. 3. Insert dropper/tip into nostril without touching the nasal lining. 4. Squeeze bottle for drops or pump for spray while breathing in gently. 5. Repeat for other nostril. 6. Avoid sniffing forcefully. 7. Clean the dropper/tip after each use. Do not share the bottle.
5. Side Effects
Common side effects may include:
- Transient burning, stinging, or dryness of nasal mucosa
- Sneezing
- Mild headache
- Local irritation
- Cooling/tingling sensation (due to camphor/menthol)
6. Drug Interactions
| Drug | Effect | Severity |
|---|---|---|
| Monoamine Oxidase Inhibitors (MAOIs) - Phenelzine, Tranylcypromine | Risk of severe, potentially fatal hypertensive crisis due to potentiation of sympathomimetic effects. | Contraindicated |
| Other Sympathomimetics (Oral/Parenteral) - Pseudoephedrine, Ephedrine | Additive adrenergic effects, increasing risk of hypertension, tachycardia, and arrhythmias. | Major |
| Beta-blockers (non-selective) - Propranolol | Unopposed alpha-adrenergic stimulation can lead to severe hypertension and bradycardia. | Major |
| Tricyclic Antidepressants (TCAs) - Amitriptyline, Imipramine | May potentiate pressor effects of phenylephrine and naphazoline. | Moderate |
| Ergot Alkaloids - Ergotamine | Increased risk of severe vasoconstriction and ischemia. | Major |
| Antihypertensives | May decrease the efficacy of antihypertensive therapy. | Moderate |
7. Patient Counselling
- DO use exactly as directed by the physician.
- DO clean the dropper/tip after each use.
- DO blow your nose gently before application.
- DO NOT use for more than 3-5 consecutive days.
- DO NOT exceed the recommended dose or frequency.
- DO NOT share this medication with anyone.
- DO NOT use if the solution is discolored or contains particles.
- DO NOT allow the tip to touch your nose, fingers, or any surface to avoid contamination.
8. Toxicology & Storage
Overdose: Systemic: Severe hypertension, headache, vomiting, palpitations, tachycardia, arrhythmias, sweating, anxiety, insomnia, tremors. CNS: Dizziness, confusion, hallucinations, seizures (especially with camphor). Severe: Respiratory depression, coma, cardiovascular collapse. Local: Severe rebound congestion, nasal mucosa damage.
Storage: Store at room temperature (15-30°C). Protect from light and excessive heat. Do not freeze. Keep the bottle tightly closed when not in use. Keep out of reach and sight of children. Discard 30 days after opening the seal, or as per manufacturer's instructions on the pack.
