Bacterial Lysate (7mg)

Clinical Pharmacologist's Monograph

⚠️ Prescription Only: This medicine is Schedule H/H1. Do not self-medicate.

1. Clinical Overview

Bacterial Lysate (7mg) is an immunomodulator derived from the mechanical lysis of standardized bacterial strains, primarily used for the prevention of recurrent respiratory tract infections. It acts as a polyvalent bacterial antigenic stimulus, enhancing both innate and adaptive immune responses. In the Indian context, it is widely prescribed for its prophylactic role in frequently ill children and adults, particularly during seasonal changes.

OnsetDurationBioavailability
Immunological effects begin within 1-2 weeks of initiation, but full prophylactic benefit is typically observed after 2-3 months of therapy. The immunomodulatory effect can persist for several months after completing a standard 3-month course, providing protection through the respiratory infection season. Not applicable in conventional pharmacokinetic terms, as it is an orally administered antigenic preparation. Its action is local (mucosal) and systemic via gut-associated lymphoid tissue (GALT).

2. Mechanism of Action

Contains lyophilized lysates of common respiratory pathogens. Upon oral administration, antigens are taken up by M cells in the Peyer's patches of the intestinal mucosa. This stimulates dendritic cells and macrophages, leading to activation of T-helper cells (Th1) and subsequent B-cell differentiation. This results in increased production of specific secretory IgA in the respiratory mucosa, enhanced phagocytic activity of macrophages, and a balanced Th1/Th2 cytokine response, providing systemic and mucosal immunological defense.

3. Indications & Uses

  • Prophylaxis of recurrent upper and lower respiratory tract infections (e.g., rhinosinusitis, pharyngitis, bronchitis)
  • Prevention of acute exacerbations in patients with chronic bronchitis
  • Reduction in frequency of infections in frequently ill children (more than 6 episodes per year)

4. Dosage & Administration

Adult Dosage: One capsule (7mg) daily on an empty stomach, preferably in the morning. Standard course: 10 days per month for 3 consecutive months. May be repeated after a break if needed.

Administration: Swallow capsule whole with a glass of water on an empty stomach, at least 30 minutes before a meal. Do not crush or chew the capsule. If opening is necessary for pediatric use, mix the powder with a small amount of cold, non-acidic food immediately before consumption.

5. Side Effects

Common side effects may include:

  • Mild gastrointestinal discomfort (nausea, abdominal pain)
  • Headache
  • Skin rash or pruritus

6. Drug Interactions

DrugEffectSeverity
Immunosuppressants (Cyclosporine, Tacrolimus, Azathioprine)May diminish the immunostimulant effect of the bacterial lysate.Moderate
Corticosteroids (high-dose, long-term)Systemic immunosuppression may reduce efficacy.Moderate
Live Attenuated VaccinesTheoretical risk of interference; maintain a gap of 2-4 weeks. No issue with inactivated vaccines.Low

7. Patient Counselling

  • Do complete the full 3-month course for optimal protection.
  • Do take the capsule on an empty stomach.
  • Do not take with hot beverages.
  • Do not use to treat an active, acute infection; it is a preventive medicine.
  • Inform your doctor if you have an autoimmune disease.

8. Toxicology & Storage

Overdose: No reports of acute overdose. Potential symptoms may include intensified common side effects like GI upset, headache, or fever.

Storage: Store below 25°C in a cool, dry place. Protect from light and moisture. Keep the blister strips in the outer carton. Keep out of reach of children.
