📋 Clinical Overview

A fixed-dose combination of proteolytic enzymes (Bromelain and Trypsin) and a bioflavonoid (Rutoside) used primarily for its anti-inflammatory, anti-edematous, fibrinolytic, and venotonic properties. It is a widely prescribed oral enzyme therapy in India for managing post-operative and post-traumatic inflammation, swelling, and pain, as well as venous disorders.

💊 Dosage & Administration

Adult: One tablet (Bromelain 180mg + Trypsin 96mg + Rutoside 200mg) two to three times daily. For acute conditions: Typically TID for 5-7 days. For chronic conditions: May be reduced to BD.

Note: Take on an empty stomach, at least 30 minutes before a meal or 2 hours after a meal, with a full glass of water. Do not crush or chew enteric-coated tablets. Swallow whole.

⚠️ Contraindications

  • Known hypersensitivity to Bromelain, Trypsin, Rutoside, pineapple, or any component of the formulation
  • Patients with severe coagulation disorders (e.g., hemophilia)
  • Patients on anticoagulant therapy with active bleeding (relative, but often considered absolute)

🔬 Mechanism of Action

The combination exerts synergistic anti-inflammatory, anti-edematous, fibrinolytic, and analgesic effects. Bromelain and Trypsin are proteolytic enzymes that break down inflammatory mediators (e.g., bradykinin), fibrin deposits, and exudative proteins, reducing capillary permeability and improving tissue drainage. Rutoside reduces capillary fragility and permeability by inhibiting phosphodiesterase, increasing cAMP, and providing antioxidant protection to the vascular endothelium.

  • Mild gastrointestinal discomfort (dyspepsia, nausea)
  • Diarrhea or loose stools
  • Allergic skin reactions (rash, itching, urticaria) - more common in pineapple-allergic individuals

🤰 Special Populations

Pregnancy: Category C: Animal reproduction studies have not been conducted. Not recommended during pregnancy unless potential benefit justifies potential risk to the fetus, especially in the first trimester. Use only under strict medical supervision.

Driving: Dizziness has been reported rarely. Patients should be cautioned about driving or operating machinery if they experience dizziness.
