📋 Clinical Overview

Invert Sugar (10%) is a sterile, non-pyrogenic, hypertonic intravenous solution containing a mixture of equal parts of dextrose and fructose (levulose). It is a readily metabolizable carbohydrate source that provides calories and fluid. Unlike dextrose alone, it is metabolized independently of insulin, making it advantageous in conditions of insulin resistance or stress. It is primarily used for parenteral nutrition and fluid/calorie replacement in patients unable to tolerate oral feeding.

💊 Dosage & Administration

Adult: Usual dose: 500 mL to 3000 mL per day. Infusion rate: Should not exceed 0.5 g of fructose per kg body weight per hour (e.g., ~500 mL of 10% solution per hour for a 70 kg adult). Total daily caloric intake from carbohydrates should be individualized.

Note: For intravenous use only. Must be administered via a central or large peripheral vein to minimize risk of thrombophlebitis. Use sterile technique. The solution is hypertonic and should be infused slowly. Check for clarity and seal integrity before use. Do not administer if particulate matter or discoloration is present.

⚠️ Contraindications

  • Known hypersensitivity to invert sugar or any component
  • Patients with hereditary fructose intolerance (HFI) due to deficiency of aldolase B
  • Anuria or severe oliguria where fluid restriction is critical

🔬 Mechanism of Action

Provides a source of carbohydrates (dextrose and fructose) for metabolic energy production. Dextrose is metabolized via the glycolytic pathway and the citric acid cycle, requiring insulin for cellular uptake in most tissues. Fructose is metabolized independently of insulin, primarily in the liver via the fructokinase pathway, making it a useful energy source during states of insulin resistance, stress, or trauma.

🤰 Special Populations

Pregnancy: Category C: Animal reproduction studies have not been conducted. Should be used during pregnancy only if clearly needed. Can be used for nutritional support in hyperemesis gravidarum.

Driving: No effect on ability to drive and use machines.
