1. Clinical Overview
Tretinoin is a first-generation topical retinoid, a derivative of Vitamin A (all-trans retinoic acid). It is a cornerstone therapy in dermatology, primarily used for its comedolytic, anti-inflammatory, and skin-normalizing properties. In the Indian market, it is widely prescribed for acne vulgaris and photoaging. It works by binding to specific nuclear receptors, modulating gene expression, and normalizing keratinocyte differentiation and desquamation.
| Onset | Duration | Bioavailability |
|---|---|---|
| Clinical improvement in acne is typically seen within 2-3 weeks, but optimal results may take 6-12 weeks of continuous use. | The therapeutic effect is not sustained after discontinuation; maintenance therapy is often required. | Topical bioavailability is low, estimated at <10% percutaneously. Systemic absorption is minimal when applied to intact skin as recommended. |
2. Mechanism of Action
Tretinoin binds to specific nuclear retinoic acid receptors (RAR-α, RAR-β, RAR-γ) within keratinocytes and sebocytes. This ligand-receptor complex then binds to retinoic acid response elements (RAREs) in DNA, modulating the transcription of genes involved in cell proliferation, differentiation, and inflammation. The net effect is normalization of follicular keratinization, reduction of microcomedone formation (comedolysis), promotion of desquamation, and inhibition of neutrophilic infiltration.
3. Indications & Uses
- Acne Vulgaris (comedonal, papulopustular)
- Photoaging (fine wrinkles, mottled hyperpigmentation, rough skin)
4. Dosage & Administration
Adult Dosage: Apply a thin film once daily, in the evening, to clean, dry affected areas. Start with alternate-day application for first 2-4 weeks to minimize irritation.
Administration: 1. Wash face with a mild, non-medicated cleanser and pat dry. 2. Wait 20-30 minutes for skin to dry completely. 3. Apply a pea-sized amount, dotting on forehead, cheeks, nose, and chin, then spread gently. 4. Avoid eyes, eyelids, nostrils, mouth, and mucous membranes. 5. Do not apply to broken, sunburned, or eczematous skin. 6. Use a non-comedogenic moisturizer to manage dryness. 7. Apply broad-spectrum sunscreen SPF 30+ every morning.
5. Side Effects
Common side effects may include:
- Skin irritation (burning, stinging)
- Dryness and scaling (xerosis)
- Erythema (redness)
- Peeling and flaking
- Initial worsening of acne ('purging') in first 3-6 weeks
6. Drug Interactions
| Drug | Effect | Severity |
|---|---|---|
| Other Topical Medications (Benzoyl Peroxide, Salicylic Acid, Sulfur, Resorcinol) | Increased risk of severe skin irritation and dryness. | Major |
| Topical Antibiotics (Clindamycin, Erythromycin) | Often used in combination but may increase irritation initially. Some fixed-dose combinations are stable. | Moderate |
| Systemic Retinoids (Isotretinoin, Acitretin) | Profoundly increases risk of retinoid toxicity (severe xerosis, cheilitis, epistaxis). Contraindicated. | Major |
| Photosensitizing Drugs (Thiazides, Tetracyclines, Fluoroquinolones, Phenothiazines) | Markedly increases risk of severe sunburn and phototoxicity. | Major |
| Abrasive Soaps, Cleansers, and Cosmetics with high alcohol/astringent content | Exacerbates drying and irritating effects. | Moderate |
7. Patient Counselling
- DO use a pea-sized amount for the entire face.
- DO apply to clean, completely dry skin.
- DO use a non-comedogenic moisturizer daily.
- DO apply broad-spectrum sunscreen SPF 30+ every morning, even on cloudy days.
- DON'T apply more than once daily; more is not better.
- DON'T apply near eyes, nostrils, or mouth.
- DON'T use abrasive scrubs, harsh soaps, or astringents.
- DON'T wax treated areas to avoid skin stripping.
8. Toxicology & Storage
Overdose: Topical overdose manifests as severe erythema, peeling, burning, stinging, and skin discomfort. Accidental oral ingestion may lead to symptoms of hypervitaminosis A: headache, nausea, vomiting, dizziness, cheilitis, and skin desquamation.
Storage: Store at room temperature (15°C to 30°C), protected from light and heat. Do not freeze. Keep the tube tightly closed. Keep out of reach of children.
