1. Clinical Overview

Tapentadol is a centrally-acting synthetic analgesic with a dual mechanism of action: mu-opioid receptor (MOR) agonism and norepinephrine reuptake inhibition (NRI). It is classified as a Schedule H1 drug in India, indicating a high potential for abuse and dependence. It is used for the management of moderate to severe acute and chronic pain, including neuropathic pain associated with diabetic peripheral neuropathy. Its unique dual mechanism may offer a favorable side effect profile, particularly regarding gastrointestinal tolerability, compared to classical opioids.

2. Mechanism of Action

Tapentadol exerts its analgesic effect through a dual mechanism: 1) Agonist activity at the mu-opioid receptor (MOR), which modulates pain perception in the central nervous system. 2) Inhibition of norepinephrine reuptake (NRI), which enhances descending inhibitory pain pathways in the spinal cord. This combination is synergistic and allows for effective analgesia with potentially lower MOR-mediated side effects.

3. Indications & Uses

  • Moderate to severe acute pain (e.g., post-operative pain)
  • Chronic pain management (e.g., osteoarthritis, low back pain)
  • Neuropathic pain associated with diabetic peripheral neuropathy (DPN)

4. Dosage & Administration

Adult Dosage: For acute pain: 50-100 mg every 4-6 hours as needed for pain. The first dose can be 50-100 mg. Maximum daily dose: 600-700 mg. For chronic neuropathic pain associated with DPN: Start at 50 mg twice daily. May be titrated to 100 mg twice daily based on tolerability and pain relief.

Administration: Tablet can be taken with or without food. Swallow whole with a glass of water. Do not crush, chew, or break the tablet. Dosing interval: Every 4-6 hours for immediate-release formulations. For missed dose, take as soon as remembered if needed for pain; do not double the dose.

7. Patient Counselling

  • DO take the medication exactly as prescribed by your doctor.
  • DO inform all your healthcare providers that you are taking tapentadol.
  • DO store the medicine in a safe place, out of reach of children and pets.
  • DO NOT crush, chew, or break the tablet.
  • DO NOT suddenly stop taking the medicine after long-term use; your doctor will taper the dose to avoid withdrawal symptoms.
  • DO NOT share your medicine with anyone else.
  • DO NOT drink alcohol while on this medication.

8. Toxicology & Storage

Overdose: Symptoms of overdose are an extension of its pharmacological effects and include: pinpoint pupils, respiratory depression (which can progress to Cheyne-Stokes respiration, apnea), extreme somnolence progressing to stupor or coma, skeletal muscle flaccidity, cold and clammy skin, bradycardia, hypotension, circulatory collapse, cardiac arrest, and death.

Storage: Store at room temperature (15°C to 30°C). Protect from moisture and light. Keep in the original container, tightly closed. Keep out of reach of children and pets. Do not flush unused medication. Dispose of unused tablets as per local regulations or through a medicine take-back program.
