1. Clinical Overview

Sodium picosulfate is a locally acting stimulant laxative of the diphenyl methane derivative class. It is a prodrug that is activated by colonic bacteria to its active metabolite, bis-(p-hydroxyphenyl)-pyridyl-2-methane (BHPM), which directly stimulates the colonic mucosa, increasing peristalsis and promoting bowel evacuation. It is widely used for bowel cleansing prior to colonoscopy and for the treatment of functional constipation.

2. Mechanism of Action

Sodium picosulfate is a prodrug. After oral administration, it passes unchanged through the stomach and small intestine. In the colon, it is hydrolyzed by bacterial sulfatase enzymes to its active form, BHPM. This active metabolite directly stimulates the sensory nerve endings in the colonic mucosa (submucosal plexus), which triggers the release of acetylcholine and prostaglandins. This leads to increased colonic peristalsis, enhanced secretion of water and electrolytes into the colonic lumen, and inhibition of water reabsorption, collectively resulting in bowel evacuation.

3. Indications & Uses

  • Bowel cleansing prior to colonoscopy or radiological examination (as part of a split-dose regimen with electrolytes)
  • Treatment of occasional (acute) constipation
  • Management of functional constipation

4. Dosage & Administration

Adult Dosage: For Constipation: 2.5 mg to 10 mg (5ml to 20ml of 2.5mg/5ml solution) once daily, preferably at bedtime. For Bowel Preparation: As per specific bowel prep kit protocol. Typically, two separate doses: first dose evening before procedure, second dose morning of procedure (split-dose regimen). Dose is usually 10mg per dose dissolved in water.

Administration: For oral use only. Measure dose accurately using the provided measuring cup or oral syringe. Can be taken with or without food, but for predictable timing, take at a consistent time (e.g., bedtime). For bowel prep, mix the measured dose in a full glass (150-200ml) of clear water, stir, and drink immediately. Follow with additional clear fluids as directed. Do not take within 1 hour of antacids or antibiotics.

7. Patient Counselling

  • DO take the exact dose prescribed by your doctor.
  • DO drink plenty of clear fluids (water, clear soups, electrolyte drinks) when using this medicine, especially for bowel prep.
  • DO take it at bedtime if prescribed for constipation to achieve a morning bowel movement.
  • DON'T use it for more than 7 days for constipation without consulting your doctor.
  • DON'T take it if you have severe stomach pain, nausea, or vomiting.
  • DON'T take other laxatives simultaneously unless advised by your doctor.
  • DO inform your doctor if you are pregnant, planning pregnancy, or breastfeeding.

8. Toxicology & Storage

Overdose: Symptoms are an extension of its pharmacological effects: severe diarrhea leading to profound dehydration, electrolyte disturbances (hypokalemia, hyponatremia), abdominal cramps, weakness, dizziness, syncope, and in extreme cases, cardiac arrhythmias or renal failure.

Storage: Store below 30°C. Protect from light. Keep the bottle tightly closed. Keep out of reach of children. Do not use after the expiry date printed on the label. Do not freeze.
