1. Clinical Overview

Rotavirus vaccine (live attenuated, oral) is a live, attenuated viral vaccine administered orally to prevent rotavirus gastroenteritis, a leading cause of severe, dehydrating diarrhea in infants and young children globally and in India. It contains weakened strains of rotavirus that replicate in the intestine to induce a protective immune response without causing significant disease. The vaccine is a critical component of the Universal Immunization Programme (UIP) in India.

2. Mechanism of Action

The vaccine contains live, attenuated rotavirus strains. Upon oral administration, these viruses replicate in the epithelial cells of the small intestine. This localized replication stimulates the production of serum neutralizing antibodies (IgG, IgA) and mucosal IgA antibodies. These antibodies provide protection against future wild-type rotavirus infection by preventing viral attachment and entry into host cells, thereby mitigating the severity of gastroenteritis.

3. Indications & Uses

  • Active immunization of infants from 6 weeks of age for the prevention of rotavirus gastroenteritis caused by the G1, G2, G3, G4, and G9 types (for pentavalent) or the G1P[8] type (for monovalent).
  • Prevention of severe, dehydrating diarrhea caused by rotavirus.

4. Dosage & Administration

Adult Dosage: Not indicated for use in adults.

Administration: For oral use only. DO NOT inject. Administer directly into the infant's mouth (inside of the cheek) using the supplied oral applicator. Can be given before, after, or concurrently with other childhood vaccines (OPV, IPV, DPT, Hepatitis B, Hib, PCV). Do not mix with any other substance or feed. Breastfeeding can be continued as usual before and after vaccination.

5. Side Effects

Common side effects may include:

  • Irritability/Fussiness
  • Mild, transient diarrhea
  • Mild, transient vomiting
  • Loss of appetite
  • Low-grade fever
  • Fatigue

7. Patient Counselling

  • DO complete the full vaccine series (2 or 3 doses) as per schedule for optimal protection.
  • DO inform the doctor if the infant has any immunodeficiency or chronic illness.
  • DO maintain routine breastfeeding before and after vaccination.
  • DONT administer the vaccine if the infant has severe diarrhea or vomiting on the day of vaccination.
  • DONT repeat a dose if the infant spits or regurgitates some of the vaccine.
  • DONT give any other oral medication (e.g., polio drops) within 15 minutes before or after the vaccine.

8. Toxicology & Storage

Overdose: There is no experience with overdose. Administration of more than the recommended number of doses is not advised but is unlikely to cause significant adverse effects beyond those listed.

Storage: Store at +2°C to +8°C in a refrigerator. DO NOT FREEZE. Protect from light. The diluent can be stored separately at room temperature. Once reconstituted (for lyophilized vaccines like ROTASIIL), use immediately. For liquid formulations (ROTAVAC, ROTARIX), use directly from the vial. Do not use if frozen or past expiry date.
