1. Clinical Overview

A live, attenuated, oral vaccine containing a monovalent human rotavirus strain (G1P[8]) for the active immunization of infants against gastroenteritis caused by rotavirus infection. It is a critical component of India's Universal Immunization Programme (UIP) to reduce the significant burden of severe, dehydrating diarrhea in children under five.

2. Mechanism of Action

Contains live, attenuated human rotavirus strain. Following oral administration, the virus replicates in the small intestine, mimicking a natural infection but without causing significant disease. This replication stimulates the production of serum neutralizing antibodies (IgG, IgA) and, more importantly, local mucosal IgA antibodies in the gut. These antibodies provide protection against subsequent wild-type rotavirus infection by preventing viral attachment and entry into enterocytes.

3. Indications & Uses

  • Active immunization of infants from 6 weeks of age for the prevention of gastroenteritis caused by Rotavirus.

4. Dosage & Administration

Adult Dosage: Not indicated for use in adults.

Administration: FOR ORAL USE ONLY. DO NOT INJECT. The vaccine is a pale yellow to pink liquid supplied in a squeezable plastic container or a tube. Administer directly into the infant's mouth towards the inner cheek. Do not mix with any other liquid or food. If the infant spits out or regurgitates most of the dose, a single replacement dose may be given at the same visit. The vaccine can be given concurrently with other childhood vaccines (OPV, IPV, DPT, Hepatitis B, Hib, PCV) at different injection sites.

5. Side Effects

Common side effects may include:

  • Irritability/Fussiness
  • Loss of appetite
  • Mild, transient diarrhea
  • Mild vomiting
  • Low-grade fever
  • Fatigue

6. Drug Interactions

7. Patient Counselling

  • DO ensure your infant receives all doses at 6, 10, and 14 weeks as per schedule, even if mildly ill with a cold.
  • DO inform the doctor if the infant has any severe allergy, immune system problem, or history of intestinal blockage.
  • DO NOT give the vaccine if the infant is older than 15 weeks for the first dose.
  • DO NOT inject the vaccine; it is for oral use only.
  • DO continue breastfeeding normally before and after vaccination.

8. Toxicology & Storage

Overdose: There is no experience with overdose. Accidental ingestion of more than one dose at a single visit is not expected to increase the severity of adverse reactions significantly.

Storage: Store frozen at -20°C or below in a dedicated freezer. It is supplied in the frozen state. Once removed for use, the thawed vaccine can be stored at +2°C to +8°C (refrigerator) for up to 24 months (check brand-specific validity) if unopened. After opening the oral applicator, use immediately. Do not refreeze once thawed. Protect from light. Use the diluent provided with the vaccine if applicable (some brands are liquid, some are lyophilized).
