1. Clinical Overview

Recombinant Human Epidermal Growth Factor (rhEGF) is a topical biological agent that mimics the endogenous EGF, a 53-amino acid polypeptide. It is a potent mitogen for epithelial cells, fibroblasts, and keratinocytes, playing a crucial role in wound healing by stimulating cell proliferation, migration, and differentiation. In the Indian market, it is primarily formulated as a gel or cream for topical application in wound management.

2. Mechanism of Action

rhEGF is a ligand for the Epidermal Growth Factor Receptor (EGFR), a transmembrane tyrosine kinase receptor. Binding induces receptor dimerization and autophosphorylation, activating major intracellular signaling cascades (RAS-RAF-MEK-ERK and PI3K-AKT-mTOR pathways). This leads to increased DNA synthesis, transcription of genes involved in cell cycle progression, and synthesis of proteins and extracellular matrix components essential for tissue repair.

3. Indications & Uses

  • Diabetic Foot Ulcers (Grade 1 & 2 as per Wagner's classification)
  • Venous Stasis Ulcers
  • Pressure Sores (Stage II & III)
  • Burn Wounds (partial thickness, after acute phase)
  • Traumatic Skin Injuries and Abrasions

4. Dosage & Administration

Adult Dosage: Apply a thin layer (approx. 0.5 gm of gel per 10 sq cm wound area) once or twice daily, or as directed by the physician. The wound should be cleaned and debrided before each application.

Administration: 1. Cleanse wound thoroughly with normal saline or prescribed antiseptic. 2. Pat dry gently. 3. Apply a uniform thin layer to cover entire wound bed. 4. Cover with a sterile non-adherent dressing. 5. Wash hands before and after application. Do not use occlusive dressings unless advised.

5. Side Effects

Common side effects may include:

  • Transient mild burning or stinging at application site
  • Localized erythema (redness)
  • Itching (pruritus)

7. Patient Counselling

  • DO clean the wound as instructed before every application.
  • DO use a clean applicator or glove to apply the gel.
  • DO keep the medicine refrigerated (2-8°C). The tube in use can be kept at room temperature (below 25°C) for up to 30 days.
  • DON'T apply the gel to infected wounds without concurrent antibiotic therapy.
  • DON'T use occlusive dressings unless specifically told by your doctor.
  • DON'T stop using the medicine even if healing seems rapid; complete the prescribed course.

8. Toxicology & Storage

Overdose: Topical overdose is unlikely to cause systemic symptoms. Excessive local application may lead to increased frequency of local side effects like burning, pain, and hypergranulation tissue.

Storage: Store unopened tubes in a refrigerator between 2°C and 8°C. Do not freeze. Protect from light. Once opened, the tube can be kept at room temperature (not exceeding 25°C) for a period of up to 30 days. Keep out of reach of children. Do not use after the expiry date.
