A fixed-dose combination (FDC) of Racecadotril, an enkephalinase inhibitor with antisecretory action, and Ofloxacin, a second-generation fluoroquinolone antibiotic. This combination is primarily indicated for the treatment of acute infectious diarrhea with a dual mechanism: Ofloxacin eradicates the causative bacterial pathogens, while Racecadotril reduces the hypersecretion of water and electrolytes into the intestinal lumen, providing symptomatic relief without affecting intestinal motility. It is a widely used FDC in the Indian market for managing acute diarrheal episodes, though its use is subject to regulatory scrutiny regarding antibiotic misuse.

2. Mechanism of Action
The combination works synergistically. Ofloxacin acts by inhibiting bacterial DNA gyrase (topoisomerase II) and topoisomerase IV, enzymes essential for DNA replication, transcription, and repair, leading to bactericidal activity against enteric pathogens. Racecadotril, after conversion to thiorphan, inhibits membrane-bound enkephalinase in the intestinal epithelium. This inhibition increases the local concentration of endogenous enkephalins, which then activate delta-opioid receptors. This activation stimulates absorption and reduces hypersecretion of water and electrolytes into the gut lumen via inhibition of adenylate cyclase.
3. Indications & Uses
- Acute infectious diarrhea of suspected or proven bacterial etiology (e.g., Traveler's diarrhea)
- Acute secretory diarrhea with signs of bacterial infection (fever, blood/mucus in stool)
4. Dosage & Administration
Adult Dosage: One tablet (Racecadotril 10mg + Ofloxacin 50mg) twice daily. Typically prescribed for 3-5 days. Do not exceed 5 days of therapy without reassessment.
Administration: Administer with or without food. Swallow whole with a full glass of water. Maintain adequate hydration with Oral Rehydration Solution (ORS) throughout treatment. Do not take with dairy products, calcium-fortified juices, or antacids containing divalent cations (Ca, Mg, Al, Fe) as they impair Ofloxacin absorption; separate by at least 2 hours.

7. Patient Counselling
- DO complete the full prescribed course even if you feel better.
- DO drink plenty of fluids, preferably ORS, to prevent dehydration.
- DO take the tablet at evenly spaced intervals (e.g., morning and evening).
- DO separate dose from dairy products, antacids, or vitamins/minerals by at least 2 hours.
- DONT take with other antidiarrheal medications like Loperamide unless advised by doctor.
- DONT share this medication with others, even if symptoms seem similar.
- DONT use for simple, non-infectious diarrhea or viral diarrhea.
8. Toxicology & Storage
Overdose: Symptoms may include exacerbation of known side effects: Severe nausea, vomiting, dizziness, headache, confusion, seizures (from Ofloxacin), and possibly acute renal failure. Prolonged QT interval and arrhythmias are possible.
Storage: Store below 30°C, in a cool, dry place. Protect from light and moisture. Keep out of reach of children. Do not use after the expiry date printed on the pack.
