Pregabalin (75mg) + Methylcobalamin (750mg) + Vitamin B6 (Pyridoxine) (1.5mg) + Folic Acid (0.75mg) (Medical Monograph)

Overview

A fixed-dose combination product containing Pregabalin, an anticonvulsant and neuropathic pain agent, combined with Methylcobalamin (active Vitamin B12), Pyridoxine (Vitamin B6), and Folic Acid. It is primarily used for the management of neuropathic pain, particularly diabetic peripheral neuropathy, with the B-vitamins intended to support nerve health and function.

Uses

  • Management of neuropathic pain associated with Diabetic Peripheral Neuropathy (DPN)
  • Adjunctive therapy for neuropathic pain conditions
  • Nutritional supplementation in conditions with suspected B-vitamin deficiency contributing to neuropathy

Side Effects

  • Dizziness
  • Somnolence (drowsiness)
  • Peripheral edema
  • Weight gain
  • Headache
  • Dry mouth
  • Blurred vision

Detailed Pharmacology

Pregabalin is a structural derivative of GABA but does not act on GABA receptors. It binds with high affinity to the alpha2-delta subunit of presynaptic voltage-gated calcium channels, inhibiting calcium influx. This reduces the release of several excitatory neurotransmitters (e.g., glutamate, noradrenaline, substance P) in hyperexcited neurons, leading to analgesic, anxiolytic, and anticonvulsant effects. Methylcobalamin is essential for the synthesis of myelin and maintenance of neuronal integrity. Pyridoxine (B6) is a cofactor in amino acid and neurotransmitter synthesis. Folic Acid is involved in nucleotide synthesis and homocysteine metabolism, potentially reducing neurotoxic effects of elevated homocysteine.
