1. Clinical Overview

Pramoxine hydrochloride is a topical, non-ester, non-amide local anesthetic agent. It is structurally distinct from the 'caine' anesthetics (like lidocaine or benzocaine) as it is a morpholine derivative. It is primarily used for its surface anesthetic and antipruritic (anti-itch) properties on skin and mucous membranes. Its mechanism involves reversible blockade of sodium ion channels in neuronal membranes, preventing the initiation and conduction of nerve impulses, thereby producing a localized loss of sensation, particularly for pain and itching.

2. Mechanism of Action

Pramoxine hydrochloride acts by reversibly binding to and inhibiting voltage-gated sodium channels on the neuronal cell membrane. This blockade prevents the transient influx of sodium ions (Na+) required for the depolarization phase of the action potential. Consequently, the initiation and propagation of electrical impulses along sensory nerve fibers are inhibited. This results in a reversible loss of sensation (anesthesia) in the localized area of application, specifically affecting pain (nociception) and itch (pruritus) sensations.

3. Indications & Uses

  • Symptomatic relief of pain and itching associated with anogenital pruritus (e.g., hemorrhoids, anal fissures)
  • Temporary relief of pain and itching from minor skin conditions such as insect bites, minor burns, sunburn, poison ivy/oak, eczema, and dermatitis
  • Relief of discomfort from episiotomy and vaginal/perianal surgery

4. Dosage & Administration

Adult Dosage: Apply a thin layer to the affected area 3 to 4 times daily, or as directed by a physician. For hemorrhoidal use: apply externally to the anal area up to 5 times daily, especially after each bowel movement.

Administration: 1. Clean and dry the affected area thoroughly. 2. Apply a small amount (pea-sized to finger-tip unit) and rub gently until absorbed. 3. For hemorrhoidal creams/ointments: apply externally only. Do not insert into rectum unless formulation is specifically designed for internal use (e.g., suppository). 4. Wash hands before and after application. 5. Avoid contact with eyes, mouth, nose, and other mucous membranes unless directed. 6. Do not bandage or wrap the treated area tightly unless advised by a doctor.

5. Side Effects

Common side effects may include:

  • Transient mild stinging, burning, or tingling at the application site
  • Localized erythema (redness)
  • Dryness or flaking of skin

7. Patient Counselling

  • DO use only on the skin or external anal area as directed.
  • DO clean the area before application.
  • DO use the smallest amount needed for relief.
  • DO wash hands after application.
  • DON'T apply to large areas of broken, blistered, or severely injured skin.
  • DON'T use with tight bandages or wraps unless prescribed.
  • DON'T get the product into your eyes, nose, mouth, or other sensitive mucous membranes.
  • DON'T use for more than 7 days unless directed by your doctor.

8. Toxicology & Storage

Overdose: Systemic toxicity is extremely rare with topical use but could occur from ingestion or massive application to damaged skin. Symptoms include CNS excitation (restlessness, tremors, seizures) followed by depression (drowsiness, coma), cardiovascular depression (bradycardia, hypotension, cardiac arrest), and methemoglobinemia (cyanosis, dyspnea).

Storage: Store at room temperature (15°C to 30°C). Protect from light and excessive heat. Keep the tube or container tightly closed. Keep out of reach of children. Do not freeze.
