1. Clinical Overview

Povidone Iodine (2% w/v) is a stable chemical complex of polyvinylpyrrolidone (povidone) and elemental iodine, providing sustained release of iodine. It is a broad-spectrum topical antiseptic and disinfectant widely used in the Indian healthcare system for its potent microbicidal activity against bacteria, viruses, fungi, protozoa, and spores. It is a cornerstone in infection prevention protocols, particularly for skin preparation before surgery, wound management, and mucous membrane disinfection.

2. Mechanism of Action

Povidone iodine acts as a reservoir, slowly releasing free iodine (I2) in aqueous solution. The free iodine penetrates the cell wall of microorganisms and oxidizes key cellular components such as nucleotides, fatty acids, and amino acids. This leads to denaturation and precipitation of proteins, disruption of microbial electron transport, and ultimately cell death.

3. Indications & Uses

  • Pre-operative skin disinfection (surgical scrubs and paints)
  • Disinfection of minor cuts, wounds, and abrasions
  • Prevention and treatment of infections in burns and ulcers
  • Disinfection of mucous membranes (e.g., vaginal disinfection pre-procedures)

4. Dosage & Administration

Adult Dosage: Apply topically to affected area 1-3 times daily. For surgical scrub: Apply for 5 minutes. For wound disinfection: Apply undiluted or as directed. Gargle: 10-20 ml of diluted solution (usually 1:10 with water) 2-3 times daily.

Administration: For intact skin: Apply and allow to dry. For wounds: Clean wound first, apply solution, may cover with sterile dressing. Do not use occlusive dressings over it. For gargle: Always dilute as per product instructions (typically 10-20 drops in a glass of water). Avoid contact with eyes. Rinse with water if accidental eye contact occurs.

5. Side Effects

Common side effects may include:

  • Transient stinging or burning at application site
  • Brown staining of skin, hair, and fabrics
  • Skin dryness or irritation

7. Patient Counselling

  • DO use only on the affected area as directed.
  • DO dilute the solution for use as a mouthwash/gargle as instructed on the label.
  • DO allow it to dry completely on skin before dressing.
  • DO store in a cool place, away from light.
  • DON'T apply to deep, puncture wounds or animal bites without medical advice.
  • DON'T use on large areas of broken skin for prolonged periods.
  • DON'T cover with airtight (occlusive) dressings.
  • DON'T use if the solution turns from brown to colorless, as it may indicate loss of potency.

8. Toxicology & Storage

Overdose: Topical overdose is unlikely. Systemic toxicity from absorption presents as: Gastrointestinal: Nausea, vomiting, diarrhea, abdominal pain. Metabolic: Metabolic acidosis, hypernatremia. Renal: Acute kidney injury. Thyroid: Hypothyroidism or hyperthyroidism. Neurologic: Delirium, stupor.

Storage: Store in tightly closed original container at room temperature (15-30°C), away from direct sunlight and heat. Keep out of reach of children. Do not freeze. Discard if solution changes color (from brown to yellow or colorless) or becomes cloudy.
