1. Clinical Overview
Pancreatin is a porcine-derived pancreatic enzyme concentrate standardized to contain lipase, amylase, and protease activities. The 180mg strength typically provides approximately 10,000-12,000 USP units of lipase, 30,000-36,000 USP units of amylase, and 10,000-12,000 USP units of protease. It is a digestive enzyme replacement therapy used to correct exocrine pancreatic insufficiency (EPI). In the Indian context, it is widely used for both pancreatic and non-pancreatic maldigestion syndromes.

2. Mechanism of Action
Pancreatin contains a mixture of porcine-derived digestive enzymes: lipase, amylase, and protease. It acts as a substitute for deficient endogenous pancreatic secretions. Lipase hydrolyzes triglycerides into monoglycerides and free fatty acids. Amylase hydrolyzes starch into dextrins and sugars. Protease hydrolyzes proteins into peptides and amino acids. This enzymatic action facilitates the normal digestion and absorption of fats, carbohydrates, and proteins from the diet.

4. Dosage & Administration
Adult Dosage: Individualized based on fat intake and severity of EPI. General starting dose: 1 to 2 capsules (180mg each) with each main meal and 1 capsule with substantial snacks. Dosage is titrated to control steatorrhea and symptoms, often ranging from 4 to 12 capsules (720mg to 2160mg) per day.
Administration: Must be taken WITH meals or snacks. Swallow capsules whole with a full glass of water. Do not crush or chew enteric-coated formulations. If capsule cannot be swallowed, it may be opened and the microspheres/microtablets sprinkled on a small amount of soft, acidic food (pH < 4.5 like applesauce, banana, yogurt) at the time of consumption. Do not mix with alkaline foods or hold in mouth. Do not mix microspheres into baby formula or milk. Take doses evenly distributed throughout the meal.

7. Patient Counselling
- DO take the capsule with every meal or substantial snack.
- DO swallow the capsule whole with plenty of water.
- DO NOT crush, chew, or hold the capsule or microspheres in your mouth.
- DO NOT take the medicine on an empty stomach.
- If you cannot swallow the capsule, you may open it and mix the tiny beads with a small spoonful of acidic, soft food (like applesauce, yogurt, banana) and swallow immediately without chewing. Do not mix with hot food.
- DO NOT mix the beads with milk, formula, or alkaline foods.
- DO NOT store opened capsules; use immediately.
8. Toxicology & Storage
Overdose: Very high doses may cause nausea, vomiting, diarrhea, abdominal cramps, and hyperuricemia. In cystic fibrosis patients, chronic overdose (>10,000 lipase units/kg/day) is associated with fibrosing colonopathy, a serious condition with stricture formation.
Storage: Store in a cool, dry place below 25°C. Protect from moisture. Keep the container tightly closed. Do not refrigerate. Keep out of reach of children.
