1. Clinical Overview

Oxymetazoline is a potent, long-acting, direct-acting sympathomimetic amine. It is a selective alpha-2 adrenergic receptor agonist with some alpha-1 activity. In the Indian context, it is primarily used as a topical nasal decongestant in the form of nasal drops or sprays. The 0.5mg/ml (0.05% w/v) concentration is the standard for intranasal use. It causes vasoconstriction of the dilated arterioles in the nasal mucosa, leading to reduced tissue hyperemia, edema, and nasal congestion. It is a first-line symptomatic treatment for allergic rhinitis, common cold, and sinusitis.

2. Mechanism of Action

Oxymetazoline is a direct-acting sympathomimetic agent that acts primarily on alpha-2 adrenergic receptors, with minor activity on alpha-1 receptors. When applied topically to the nasal mucosa, it stimulates these receptors on the smooth muscle walls of the blood vessels (arterioles). This stimulation leads to vasoconstriction, reducing blood flow to the engorged nasal turbinates and sinusoids. The reduction in vascular engorgement and capillary permeability decreases mucosal edema, resulting in shrinkage of the nasal mucosa and widening of the nasal airway, relieving nasal obstruction.

3. Indications & Uses

  • Symptomatic relief of nasal congestion associated with allergic rhinitis (seasonal and perennial)
  • Symptomatic relief of nasal congestion due to the common cold
  • Symptomatic relief of nasal congestion in sinusitis

4. Dosage & Administration

Adult Dosage: 2-3 drops or 2-3 sprays (0.05% solution, 0.5mg/ml) into each nostril every 10-12 hours. Do not exceed 2 doses in 24 hours.

Administration: 1. Gently blow nose to clear nostrils. 2. Tilt head slightly back. 3. Insert applicator tip into nostril, pointing away from the nasal septum. 4. Squeeze bottle for drops or pump for spray while breathing in gently through the nose. 5. Avoid touching the tip to the nasal lining to prevent contamination. 6. Do not share the bottle with others.

5. Side Effects

Common side effects may include:

  • Transient mild burning, stinging, or dryness of the nasal mucosa
  • Sneezing
  • Mild headache

7. Patient Counselling

  • DO use exactly as directed, not more frequently or for longer than 5-7 days.
  • DO clear nostrils gently before administration.
  • DO clean the applicator tip with a clean tissue after use.
  • DON'T share your nasal spray/drops with anyone.
  • DON'T tilt your head too far back to prevent the solution from running down your throat.
  • DON'T use if the solution is discolored or contains particles.
  • DON'T use to treat symptoms other than nasal congestion without consulting a doctor.

8. Toxicology & Storage

Overdose: Topical Overuse: Severe rebound nasal congestion, anxiety, insomnia, hypertension, tachycardia. Accidental Ingestion (especially in children): Profound CNS depression (lethargy, sedation, coma), bradycardia, hypotension, hypothermia, miosis, respiratory depression. Hypertension may precede hypotension.

Storage: Store at room temperature (15-30°C). Protect from light and excessive heat. Do not freeze. Keep the bottle tightly closed when not in use. Keep out of reach and sight of children.
