1. Clinical Overview
A fixed-dose combination topical preparation containing an aminoglycoside antibiotic (Neomycin) and a potent glucocorticoid (Dexamethasone). It is primarily used for the treatment of superficial bacterial infections of the eye and external ear canal, where inflammation is a significant component. Neomycin provides bactericidal action against susceptible gram-negative and some gram-positive organisms, while Dexamethasone exerts potent anti-inflammatory, anti-allergic, and immunosuppressive effects to control swelling, redness, and itching.

2. Mechanism of Action
Neomycin binds irreversibly to the 30S ribosomal subunit of susceptible bacteria, inhibiting protein synthesis and causing bactericidal effect. Dexamethasone is a potent glucocorticoid that binds to cytoplasmic glucocorticoid receptors, modulating gene transcription. It inhibits phospholipase A2, reducing the production of pro-inflammatory mediators like prostaglandins and leukotrienes, and suppresses migration of polymorphonuclear leukocytes.
3. Indications & Uses
- Bacterial conjunctivitis with significant inflammation
- Blepharitis (lid margin inflammation) with secondary bacterial infection
- External Otitis (Otitis Externa) of bacterial origin with inflammation
- Post-operative ophthalmic inflammation with risk of infection
4. Dosage & Administration
Adult Dosage: Eye: Instill 1-2 drops into the conjunctival sac every 4 to 6 hours. In severe cases, may be used hourly initially, tapering frequency. Ear: Instill 2-3 drops into the affected ear 3-4 times daily.
Administration: Shake suspension well before use. Wash hands. Do not touch dropper tip to any surface. For eyes: Tilt head back, pull lower lid down, instill drops, close eye gently for 1-2 minutes. For ears: Lie on side, instill drops, remain positioned for 5 minutes. Do not share medication. Wait at least 5-10 minutes between instilling different eye drops.
5. Side Effects
Common side effects may include:
- Transient stinging/burning sensation upon instillation
- Itching or irritation at application site
- Blurred vision (ophthalmic) temporarily
- Dryness

7. Patient Counselling
- DO use exactly as prescribed for the full prescribed length of time.
- DO shake the bottle well before use.
- DO wash hands before and after application.
- DO NOT touch the dropper tip to your eye, ear, fingers, or any surface.
- DO NOT wear contact lenses while having an eye infection or while using this medication (ophthalmic).
- DO NOT use for longer than prescribed.
- DO NOT share your medication.
- DO NOT use leftover medication for a new infection.
8. Toxicology & Storage
Overdose: Topical overdose is unlikely but could manifest as increased local side effects: severe irritation, worsening redness, pain. Accidental ingestion: Nausea, vomiting, diarrhea, ototoxicity, nephrotoxicity. Systemic steroid effects from massive topical application: Cushingoid features, hyperglycemia.
Storage: Store at room temperature (15-25°C). Protect from light. Do not freeze. Keep the bottle tightly closed. Keep out of reach of children. Do not use if the suspension is discolored or contains particles. Discard the bottle 4 weeks after opening (check manufacturer's instruction).
