1. Clinical Overview
Minocycline is a second-generation, semi-synthetic tetracycline-class antibiotic. The 65mg strength is a commonly available modified-release formulation in India, designed for once-daily dosing. It exhibits broad-spectrum bacteriostatic activity against a wide range of Gram-positive, Gram-negative, atypical, and anaerobic organisms. It is particularly valued for its superior tissue penetration, including the central nervous system, skin, and prostate, and its anti-inflammatory properties, which extend its use beyond traditional infections.

2. Mechanism of Action
Minocycline inhibits bacterial protein synthesis by reversibly binding to the 30S ribosomal subunit. This prevents the attachment of aminoacyl-tRNA to the acceptor site on the mRNA-ribosome complex, thereby inhibiting the elongation of the peptide chain.
3. Indications & Uses
- Moderate to severe inflammatory acne vulgaris
- Non-gonococcal urethritis and cervicitis caused by *Chlamydia trachomatis*
- Rickettsial infections (e.g., scrub typhus, Q fever)
- Respiratory tract infections caused by susceptible *Mycoplasma pneumoniae*
- Skin and soft tissue infections (e.g., folliculitis, cellulitis caused by *Staphylococcus aureus*)
4. Dosage & Administration
Adult Dosage: For Acne & most infections: 65mg to 100mg once daily or in divided doses (e.g., 50mg twice daily). The 65mg MR tablet is typically taken as ONE tablet once daily. For severe infections: Initial dose of 200mg, followed by 100mg every 12 hours or 65-100mg once daily.
Administration: Swallow the 65mg modified-release tablet whole with a full glass of water. Can be taken with or without food, but taking it with food may reduce the risk of gastrointestinal upset. Do NOT take with dairy products, calcium-fortified juices, antacids, or iron supplements; maintain a 2-3 hour gap. Maintain adequate fluid intake to reduce risk of esophageal irritation and ulceration.
5. Side Effects
Common side effects may include:
- Nausea, vomiting, epigastric distress, diarrhea
- Dizziness, lightheadedness, vertigo (vestibular toxicity - dose-related)
- Headache
- Skin hyperpigmentation (blue-gray/black, especially in scars or old acne sites)
- Oral candidiasis (thrush), vaginal candidiasis

7. Patient Counselling
- DO take the tablet with a full glass of water while sitting or standing upright to prevent esophageal irritation.
- DO complete the full prescribed course, even if you feel better.
- DO use sunscreen and wear protective clothing as minocycline can make your skin more sensitive to sunlight (photosensitivity).
- DO inform your doctor if you are taking birth control pills, as minocycline may reduce their effectiveness.
- DON'T take with dairy products, antacids, iron, or calcium supplements. Separate doses by at least 2-3 hours.
- DON'T give this medicine to children under 8 years old.
- DON'T use if you are pregnant or planning pregnancy.
8. Toxicology & Storage
Overdose: Symptoms are an extension of common adverse effects: severe nausea, vomiting, diarrhea, dizziness, vertigo, headache. In massive overdose, hepatotoxicity or pancreatitis may occur.
Storage: Store below 30°C, in a cool, dry place. Protect from light and moisture. Keep out of reach of children. Do not use after the expiry date printed on the pack.
