1. Clinical Overview
Miconazole is a synthetic imidazole antifungal agent used topically for the treatment of superficial fungal infections. In the 2% w/w concentration, it is primarily formulated as creams, gels, powders, and sprays for cutaneous application. It exhibits broad-spectrum fungistatic and fungicidal activity against dermatophytes (Trichophyton, Microsporum, Epidermophyton), yeasts (Candida albicans, Malassezia furfur), and other fungi. Its mechanism involves inhibition of ergosterol synthesis, a critical component of fungal cell membranes.

2. Mechanism of Action
Miconazole inhibits the fungal cytochrome P450 enzyme 14α-demethylase. This enzyme is crucial for converting lanosterol to ergosterol, the principal sterol component of the fungal cell membrane. Inhibition leads to depletion of ergosterol and accumulation of toxic methylated sterols, disrupting membrane structure and function. This results in increased membrane permeability, leakage of cellular contents, and inhibition of fungal growth.
3. Indications & Uses
- Cutaneous Candidiasis (intertrigo, paronychia)
- Dermatophytoses (Tinea corporis, Tinea cruris, Tinea pedis)
- Pityriasis versicolor (Tinea versicolor)
4. Dosage & Administration
Adult Dosage: Apply a thin layer to the affected and surrounding skin area twice daily (morning and evening). For cream/gel: Gently rub in. For powder/spray: Apply a light covering.
Administration: 1. Wash and dry the affected area thoroughly before application. 2. Apply a thin layer and rub gently. 3. Wash hands after application unless hands are being treated. 4. For intertriginous areas (skin folds), use sparingly to avoid maceration. 5. Treatment should typically continue for 2 weeks for dermatophytosis and at least 1 week after symptoms clear to prevent relapse. For pityriasis versicolor, treatment for 2-4 weeks is common.

Common side effects may include:
- Local burning or stinging sensation at application site
- Mild erythema (redness)

7. Patient Counselling
- DO wash and dry the area thoroughly before application.
- DO use the medication for the full prescribed time, even if symptoms improve earlier.
- DO wear loose-fitting, breathable clothing, especially for infections in skin folds.
- DON'T apply to eyes, nose, mouth, or inside the vagina (unless specifically formulated for vaginal use).
- DON'T cover the treated area with tight bandages or occlusive dressings unless directed by a doctor.
- DON'T share towels, clothing, or footwear to prevent spread of infection.
8. Toxicology & Storage
Overdose: Topical overdose is unlikely. Accidental ingestion of topical product may cause gastrointestinal symptoms like nausea, vomiting, diarrhea, and abdominal pain. With large oral ingestion of gel, symptoms could include CNS effects (drowsiness, hallucinations) and liver enzyme elevation.
Storage: Store at room temperature (15-25°C), protected from light and moisture. Do not freeze. Keep the tube/bottle tightly closed. Keep out of reach of children. Do not use after the expiry date printed on the pack.
