1. Clinical Overview
A fixed-dose combination (FDC) oral antidiabetic agent containing Metformin Hydrochloride (a biguanide) and Vildagliptin (a dipeptidyl peptidase-4 inhibitor). This combination provides complementary mechanisms of action to improve glycemic control in type 2 diabetes mellitus (T2DM) by reducing hepatic glucose production, increasing insulin sensitivity, and enhancing glucose-dependent insulin secretion while suppressing glucagon secretion. It is a second-line or add-on therapy when monotherapy with metformin is insufficient.

2. Mechanism of Action
The combination works via two distinct pathways. Metformin decreases hepatic gluconeogenesis, increases peripheral glucose uptake and utilization, and may delay intestinal glucose absorption. Vildagliptin inhibits the enzyme dipeptidyl peptidase-4 (DPP-4), which degrades the incretin hormones glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP). This inhibition increases active incretin levels, leading to enhanced glucose-dependent insulin secretion from pancreatic beta cells and suppressed glucagon secretion from alpha cells.
3. Indications & Uses
- Management of Type 2 Diabetes Mellitus in adults as an adjunct to diet and exercise when dual therapy with metformin and vildagliptin is appropriate.
- As second-line therapy when metformin monotherapy (at maximally tolerated dose) fails to provide adequate glycemic control.
4. Dosage & Administration
Adult Dosage: One tablet (Metformin 500mg + Vildagliptin 50mg) twice daily, with meals. The dose may be titrated based on glycemic control and tolerability, up to a maximum of Metformin 2000mg + Vildagliptin 100mg per day (e.g., two tablets twice daily of this strength).
Administration: Swallow whole with a glass of water during or immediately after meals (breakfast and dinner) to reduce gastrointestinal side effects. Do not crush or chew. Maintain adequate fluid intake.

7. Patient Counselling
- DO take the tablet twice daily with meals.
- DO regularly monitor your blood glucose levels as advised.
- DO maintain your prescribed diet and exercise regimen.
- DO inform all your doctors and the radiologist that you are on this medicine before any procedure/surgery.
- DO report any severe stomach pain, nausea, or difficulty breathing immediately.
- DONT skip meals.
- DONT consume alcohol excessively.
- DONT take the medicine if you are severely dehydrated or have a severe infection.
- DONT start this medicine if you are pregnant or planning pregnancy without consulting your doctor.
8. Toxicology & Storage
Overdose: Metformin Overdose: Hypoglycemia (especially with concomitant use of other antidiabetics), lactic acidosis (symptoms include vomiting, abdominal pain, hyperventilation, lethargy, coma). Vildagliptin Overdose: Expected to be an extension of its pharmacologic effects; limited data, but may include hypoglycemia, dizziness, and GI disturbances.
Storage: Store below 30°C. Protect from light and moisture. Keep the container tightly closed. Keep out of reach and sight of children. Do not use after the expiry date printed on the pack.
