1. Clinical Overview

Loteprednol etabonate is a site-specific, soft corticosteroid (C-20 ester corticosteroid) designed for topical ophthalmic use. It is a prodrug that undergoes rapid hydrolysis to an inactive metabolite, minimizing systemic absorption and side effects. It is highly effective in treating inflammatory and allergic conditions of the eye with a superior safety profile compared to older corticosteroids, particularly regarding intraocular pressure (IOP) elevation.

2. Mechanism of Action

Loteprednol etabonate binds to glucocorticoid receptors in ocular tissues, forming a complex that translocates to the cell nucleus. This complex modulates gene transcription, leading to the synthesis of lipocortins. Lipocortins inhibit phospholipase A2, thereby preventing the release of arachidonic acid from membrane phospholipids. This blocks the subsequent production of key inflammatory mediators like prostaglandins and leukotrienes. It also inhibits the expression of cytokines, adhesion molecules, and other mediators of the inflammatory cascade.

3. Indications & Uses

  • Treatment of post-operative inflammation following ocular surgery
  • Treatment of steroid-responsive inflammatory conditions of the palpebral and bulbar conjunctiva, cornea, and anterior segment of the globe (e.g., allergic conjunctivitis, vernal keratoconjunctivitis, episcleritis, anterior uveitis)

4. Dosage & Administration

Adult Dosage: For inflammation: 1-2 drops in the affected eye(s) 4 times daily. For post-op inflammation: 1-2 drops in the operated eye(s) 4 times daily beginning 24 hours after surgery and continuing for 2 weeks. Dosing may be tapered.

Administration: 1. Wash hands. 2. Tilt head back. 3. Gently pull lower eyelid down to form a pouch. 4. Instill prescribed number of drops without touching the dropper tip to eye or any surface. 5. Close eyes gently for 1-2 minutes. Apply gentle pressure to the nasolacrimal duct (inner corner of eye) for 1 minute to minimize systemic absorption. 6. Wait at least 5-10 minutes before instilling any other ophthalmic medication.

5. Side Effects

Common side effects may include:

  • Transient ocular burning/stinging on instillation
  • Blurred vision immediately after instillation
  • Foreign body sensation
  • Itching
  • Dry eye
  • Conjunctival hyperemia (redness)

7. Patient Counselling

  • DO wash hands before use.
  • DO apply gentle pressure on the tear duct (inner corner of eye) for 1 minute after instillation.
  • DO wait at least 5-10 minutes before using other eye drops.
  • DO NOT touch the dropper tip to any surface, including the eye.
  • DO NOT wear contact lenses while having an active inflammatory or infectious condition. Do not use while lenses are in eyes. Wait at least 15 minutes after instillation before inserting lenses.
  • DO NOT discontinue medication abruptly if used for more than 10 days without consulting your doctor.

8. Toxicology & Storage

Overdose: Topical overdose is unlikely to cause acute systemic toxicity. Excessive ocular use may lead to severe ocular side effects: marked elevation of IOP, severe corneal infiltrates, increased risk of infection, and cataract formation.

Storage: Store at controlled room temperature (15°C to 25°C). Protect from light and moisture. Keep the bottle tightly closed when not in use. Do not freeze. Keep out of reach of children. Discard the bottle 28 days after first opening to prevent contamination. Do not use if the solution is discolored or contains particles.
