1. Clinical Overview

Loteprednol etabonate is a site-specific, soft corticosteroid (C-20 ester corticosteroid) designed for topical ophthalmic use. It is a prodrug that undergoes rapid hydrolysis to an inactive metabolite after exerting its anti-inflammatory effect, minimizing systemic absorption and side effects. The 0.2% w/v concentration is primarily indicated for the short-term treatment of post-operative inflammation following ocular surgery.

2. Mechanism of Action

Loteprednol etabonate binds to glucocorticoid receptors in ocular tissues, leading to the formation of a receptor-ligand complex that translocates to the cell nucleus. This complex binds to glucocorticoid response elements (GREs) on DNA, modulating gene transcription. It inhibits the expression of pro-inflammatory cytokines (IL-1, IL-6, TNF-α), adhesion molecules (ICAM-1), and enzymes like phospholipase A2 and cyclooxygenase-2 (COX-2). This results in reduced synthesis of prostaglandins and leukotrienes, leading to decreased inflammation, edema, fibrin deposition, capillary dilation, and migration of inflammatory cells.

3. Indications & Uses

  • Treatment of post-operative inflammation following ocular surgery (e.g., cataract surgery).
  • Management of steroid-responsive inflammatory conditions of the palpebral and bulbar conjunctiva, cornea, and anterior segment of the globe (e.g., allergic conjunctivitis).

4. Dosage & Administration

Adult Dosage: 1 drop instilled into the conjunctival sac of the affected eye(s) 4 times daily. In post-operative inflammation, may start 24 hours after surgery and continue for 2 weeks.

Administration: Wash hands before use. Tilt head back, pull down lower eyelid to form a pouch. Instill drop without touching the tip to eye or any surface. Close eye gently for 1-2 minutes, applying gentle pressure to the nasolacrimal duct (inner corner of eye) to minimize systemic absorption. Wait at least 5-10 minutes before instilling any other ophthalmic medication.

5. Side Effects

Common side effects may include:

  • Transient burning or stinging on instillation.
  • Blurred vision immediately after instillation.
  • Foreign body sensation.
  • Itching.
  • Mild conjunctival hyperemia.

7. Patient Counselling

  • DO wash hands before and after use.
  • DO apply gentle pressure on the tear duct for 1-2 minutes after instillation.
  • DO wait at least 5-10 minutes before using other eye drops.
  • DO NOT touch the dropper tip to your eye, fingers, or any surface.
  • DO NOT wear contact lenses while having inflammation or while using this medication unless approved by your doctor.
  • DO NOT discontinue the medication abruptly without consulting your doctor.

8. Toxicology & Storage

Overdose: Topical overdose is unlikely to cause acute systemic toxicity. Excessive use may lead to severe local effects: marked elevation of IOP, severe corneal damage, increased risk of infection, and cataract formation.

Storage: Store at controlled room temperature (15°C to 25°C). Protect from light and moisture. Keep the bottle tightly closed when not in use. Do not freeze. Discard the bottle 4 weeks after opening. Keep out of reach of children.
