A fixed-dose combination of an osmotic laxative (Lactitol) and a bulk-forming laxative (Ispaghula) used for the effective management of chronic constipation. Lactitol draws water into the bowel, softening stools, while Ispaghula adds bulk and stimulates peristalsis, providing a synergistic, dual-mechanism action. It is particularly useful in patients with functional constipation, opioid-induced constipation, and constipation-predominant IBS (IBS-C) in the Indian population.

2. Mechanism of Action
The combination works via complementary mechanisms. Lactitol, a poorly absorbed disaccharide, exerts an osmotic effect in the colon, drawing water into the intestinal lumen, increasing stool water content, and softening the stool. Ispaghula husk, a hydrophilic colloid, absorbs water and swells to form a gelatinous mass, increasing fecal bulk. This bulk stimulates colonic peristalsis via mechanoreceptor activation, promoting bowel movement.
3. Indications & Uses

4. Dosage & Administration
Adult Dosage: One sachet (containing Lactitol 10gm + Ispaghula 3.5gm) once or twice daily, preferably after a meal. The powder must be mixed/stirred into a full glass (200-250 mL) of water or juice and consumed immediately.
Administration: 1. Empty the entire contents of the sachet into a dry glass. 2. Add at least 200-250 mL of cool water, juice, or milk. 3. Stir vigorously and drink immediately before the mixture thickens. 4. Follow with an additional half glass of water. Do not take in dry form. Administer at least 2 hours apart from other medications to avoid impaired absorption.
5. Side Effects
Common side effects may include:
- Abdominal distension
- Flatulence (excessive gas)
- Mild abdominal cramping or discomfort
- Borborygmi (stomach rumbling)

7. Patient Counselling
- DO mix the powder in a full glass of water/juice and drink immediately.
- DO drink an extra glass of water after taking the medication.
- DO maintain a diet rich in natural fiber (fruits, vegetables) and adequate fluid intake (2-3 L/day).
- DO take other medications at least 2 hours before or after this combination.
- DO NOT consume the dry powder.
- DO NOT use if you experience symptoms of intestinal blockage (severe pain, vomiting, no bowel movements).
8. Toxicology & Storage
Overdose: Symptoms of overdose include severe diarrhea, abdominal cramps, dehydration, hypokalemia, and hyponatremia. Excessive flatulence and bloating are common.
Storage: Store in a cool, dry place, below 30°C. Protect from moisture. Keep the sachets in the original packaging. Keep out of reach of children. Do not use after the expiry date printed on the pack.
