1. Clinical Overview

A fixed-dose combination (FDC) oral anti-diabetic drug (OAD) containing a sulfonylurea (Glimepiride), a biguanide (Metformin), and an alpha-glucosidase inhibitor (Voglibose). This triple-drug combination targets multiple pathophysiological defects of type 2 diabetes mellitus (T2DM) - insulin resistance, insulin deficiency, and postprandial hyperglycemia - providing synergistic glycemic control. It is indicated for patients inadequately controlled on dual therapy.

2. Mechanism of Action

This combination acts synergistically via three distinct mechanisms: 1) Glimepiride stimulates insulin secretion from pancreatic beta-cells. 2) Metformin decreases hepatic glucose production (gluconeogenesis) and improves peripheral insulin sensitivity. 3) Voglibose delays the digestion and absorption of complex carbohydrates in the small intestine, flattening the postprandial glucose spike.

3. Indications & Uses

  • Type 2 Diabetes Mellitus in adult patients inadequately controlled on any two of the following: Glimepiride, Metformin, or Voglibose (or other alpha-glucosidase inhibitor).
  • Type 2 Diabetes Mellitus as a step-up therapy from dual combination (e.g., Metformin + Glimepiride) when postprandial hyperglycemia remains uncontrolled.

4. Dosage & Administration

Adult Dosage: Usually one tablet twice daily, with the first bite of the two main meals (breakfast and dinner). The dose must be individualized based on glycemic control. Starting dose is often one tablet once daily with the main meal.

Administration: Take with the first bite of the main meals (usually breakfast and dinner). Swallow the tablet whole with a glass of water. Do not crush or chew. Adherence to a regular meal schedule is crucial to avoid hypoglycemia. Avoid excessive intake of complex carbohydrates at once.

5. Side Effects

Common side effects may include:

  • Hypoglycemia (sweating, tremor, palpitations, hunger) - primarily due to Glimepiride.
  • Gastrointestinal: Flatulence, abdominal distension, diarrhea, abdominal pain, nausea, vomiting - primarily due to Voglibose and Metformin.
  • Metallic taste (Metformin).

7. Patient Counselling

  • DO take the tablet with the first bite of your main meals (breakfast & dinner).
  • DO adhere to a regular meal schedule. Do not skip meals.
  • DO regularly monitor your blood glucose levels as advised by your doctor.
  • DO carry a fast-acting sugar source (glucose tablets, candy) to treat hypoglycemia.
  • DO inform all your doctors and dentists that you are on this medication.
  • DON'T consume alcohol while on this medication.
  • DON'T take the drug if you are fasting or during acute illness (e.g., vomiting, fever) without consulting your doctor.
  • DON'T crush or chew the tablet.
  • DON'T start any new medicine (including OTC, herbal) without consulting your doctor.

8. Toxicology & Storage

Overdose: Primarily manifests as severe, prolonged hypoglycemia (cold sweats, tremors, anxiety, blurred vision, seizures, coma). Lactic acidosis may occur with Metformin overdose (non-specific symptoms: vomiting, abdominal pain, hyperventilation, hypothermia, circulatory collapse). GI disturbances from Voglibose overdose would be pronounced.

Storage: Store below 30°C. Protect from light and moisture. Keep the blister strips in the outer carton. Keep out of reach of children. Do not use after the expiry date printed on the pack.
