A fixed-dose combination (FDC) of a thienodiazepine derivative (Etizolam) and a selective serotonin reuptake inhibitor (SSRI) (Escitalopram). Etizolam provides rapid anxiolytic, sedative, and muscle relaxant effects, while Escitalopram provides sustained antidepressant and anxiolytic effects by correcting underlying serotonergic dysfunction. This combination is used for the short-term management of severe anxiety disorders with associated depressive symptoms, leveraging the immediate relief from Etizolam and the long-term therapeutic action of Escitalopram.

2. Mechanism of Action
The combination works via two distinct but complementary mechanisms. Etizolam acts as a positive allosteric modulator at the GABA-A receptor, enhancing the inhibitory effect of GABA, leading to CNS depression (anxiolysis, sedation). Escitalopram is the pure S-enantiomer of citalopram and potently and selectively inhibits the presynaptic serotonin (5-HT) reuptake pump (SERT), increasing synaptic serotonin levels, which is implicated in mood regulation and anxiety pathways.
3. Indications & Uses
- Generalized Anxiety Disorder (GAD) with co-existing depressive symptoms
- Mixed Anxiety and Depressive Disorder (MADD) as per ICD-10/11
4. Dosage & Administration
Adult Dosage: One tablet (Etizolam 0.5mg + Escitalopram 5mg) orally once daily, preferably in the evening. Initiate at the lowest dose. The etizolam component should be used for the shortest possible duration (2-4 weeks). The escitalopram dose may be continued as monotherapy thereafter.
Administration: Take with or without food. Swallow whole with a glass of water. Do not crush or chew. Adhere strictly to the prescribed duration for the etizolam component. Do not stop abruptly; taper the etizolam under medical supervision.

7. Patient Counselling
- DO take the medicine exactly as prescribed by your doctor.
- DO NOT stop taking the medicine suddenly, especially the etizolam part. Your doctor will advise a tapering schedule.
- DO NOT increase the dose or frequency on your own.
- DO NOT consume alcohol in any form while on this medication.
- DO inform all your doctors and dentists that you are taking this medicine before any new treatment or surgery.
- DO keep all follow-up appointments with your psychiatrist.
8. Toxicology & Storage
Overdose: Manifestations of etizolam overdose (CNS depression) dominate: Severe drowsiness, confusion, lethargy, ataxia, diminished reflexes, coma, respiratory depression, hypotension. Escitalopram overdose adds risk of serotonin toxicity: Agitation, tremor, tachycardia, hyperthermia, rigidity, seizures, ECG changes (QT prolongation).
Storage: Store below 30°C, in a cool, dry place. Protect from light and moisture. Keep the bottle tightly closed. Keep out of reach of children and pets. Do not use after the expiry date printed on the pack. Dispose of unused medication safely; do not flush. Return to pharmacy for disposal if possible.
