1. Clinical Overview

A combined oral contraceptive (COC) pill containing a low-dose synthetic estrogen (Ethinyl Estradiol) and a unique progestin (Drospirenone) derived from 17α-spironolactone. It provides effective contraception and is also indicated for the treatment of moderate acne vulgaris in women seeking oral contraception. Drospirenone exhibits anti-mineralocorticoid and anti-androgenic properties, which can offer benefits like reduced water retention and improved acne.

2. Mechanism of Action

The primary contraceptive mechanism is the suppression of gonadotropin secretion (FSH and LH) from the pituitary gland, which inhibits follicular development and ovulation. It also alters the cervical mucus (making it hostile to sperm penetration) and the endometrium (making it less receptive to implantation).

3. Indications & Uses

  • Oral Contraception
  • Treatment of moderate acne vulgaris in women aged 14 years and older who have no known contraindications to oral contraceptive therapy and have achieved menarche.

4. Dosage & Administration

Adult Dosage: One tablet orally once daily, at approximately the same time each day, for 21 consecutive days followed by a 7-day tablet-free interval (or 7 placebo tablets). Menstruation usually occurs during this break. A new pack is started the next day after the 7-day break.

Administration: Take with water, with or without food. If vomiting or severe diarrhea occurs within 3-4 hours of intake, it should be considered a missed pill. For first-time users, start on Day 1 of menstrual cycle (no need for backup contraception) or on Sunday after period begins (requires backup contraception for first 7 days). Follow the specific pack instructions for missed pills.

5. Side Effects

Common side effects may include:

  • Nausea, abdominal pain
  • Headache, migraine
  • Breast pain/tenderness
  • Irregular uterine bleeding (spotting, breakthrough bleeding) especially in first 3 months
  • Mood changes, depressed mood
  • Decreased libido
  • Acne (may improve over time)

7. Patient Counselling

  • DO take the pill at the same time every day to maintain efficacy.
  • DO use a backup contraceptive method (e.g., condoms) for the first 7 days of the first pack and if you miss a pill.
  • DO inform all healthcare providers (including dentists) that you are on this pill.
  • DO report severe headaches, chest pain, leg pain/swelling, shortness of breath, or jaundice immediately.
  • DONT smoke while taking this pill, especially if you are over 35.
  • DONT take this pill if you are pregnant or suspect pregnancy.
  • DONT start new long-term medications (e.g., for epilepsy, TB, HIV) without consulting your doctor about interactions.

8. Toxicology & Storage

Overdose: Nausea, vomiting, and withdrawal bleeding in young girls. No serious sequelae expected from acute overdose.

Storage: Store below 30°C. Protect from light and moisture. Keep out of reach of children. Do not use after the expiry date printed on the pack.
