1. Clinical Overview

Clobetasol propionate is a super-high potency topical corticosteroid of the glucocorticoid class. It is one of the most potent topical steroids available, exerting potent anti-inflammatory, antipruritic, and vasoconstrictive actions. It is primarily used for short-term treatment of severe, resistant inflammatory dermatoses where less potent steroids have failed. In the Indian context, it is widely prescribed but requires careful monitoring due to its high potency and significant risk of local and systemic adverse effects, especially with prolonged use or on large body surface areas.

2. Mechanism of Action

Clobetasol propionate is a synthetic glucocorticoid receptor agonist. It diffuses across cell membranes and binds with high affinity to cytoplasmic glucocorticoid receptors. The receptor-ligand complex translocates to the nucleus, where it modulates gene transcription by binding to glucocorticoid response elements (GREs). This leads to the induction of anti-inflammatory proteins (e.g., lipocortin-1) and the repression of genes encoding pro-inflammatory mediators.

3. Indications & Uses

  • Plaque Psoriasis (short-term treatment)
  • Lichen Planus (especially hypertrophic)
  • Severe Atopic Dermatitis/Eczema (resistant cases)
  • Discoid Lupus Erythematosus (cutaneous lesions)
  • Severe Contact Dermatitis (allergic or irritant)

4. Dosage & Administration

Adult Dosage: Apply a thin film to the affected area once or twice daily. Treatment should be limited to 2 consecutive weeks. Maximum weekly dose should not exceed 50 grams.

Administration: Wash and dry the area gently. Apply a thin layer and rub in lightly. Do not apply under occlusive dressings unless specifically directed by a physician for a severe, localized condition. Wash hands after application unless hands are the treatment site. Do not use on broken skin, near eyes, or on mucous membranes.

5. Side Effects

Common side effects may include:

  • Local burning or stinging sensation
  • Skin dryness
  • Itching (pruritus) at application site
  • Erythema

7. Patient Counselling

  • DO use only as directed by your doctor.
  • DO apply a thin layer only to the affected area.
  • DO wash your hands before and after application (unless hands are being treated).
  • DO report any signs of infection (increased redness, pus, pain) to your doctor.
  • DON'T use it on your face, underarms, or groin unless specifically told to do so.
  • DON'T cover the area with tight bandages or dressings unless advised.
  • DON'T use it for longer than the prescribed duration (usually 2 weeks).
  • DON'T share your medication with others.

8. Toxicology & Storage

Overdose: Topical overdose manifests as severe local side effects: pronounced skin atrophy, striae, telangiectasia, and signs of systemic absorption: Cushingoid features (moon face, central obesity), hyperglycemia, hypertension, HPA axis suppression (weakness, fatigue, hypotension).

Storage: Store at room temperature (15-25°C), protected from light and moisture. Do not freeze. Keep the tube or container tightly closed. Keep out of reach and sight of children. Do not use after the expiry date printed on the pack.
