1. Clinical Overview

Choline Salicylate is a non-steroidal anti-inflammatory drug (NSAID) and a salicylate ester, combining choline with salicylic acid. It acts as a potent analgesic, antipyretic, and anti-inflammatory agent. In the Indian market, it is primarily formulated as a topical gel or liniment for localized pain relief, offering the advantage of direct application with minimal systemic absorption and reduced risk of gastrointestinal side effects compared to oral salicylates.

2. Mechanism of Action

Choline Salicylate exerts its effects primarily through its salicylate component. It acts by irreversibly inhibiting cyclooxygenase (COX) enzymes, COX-1 and COX-2, thereby blocking the conversion of arachidonic acid to prostaglandins (PGs) and thromboxanes. This inhibition reduces the synthesis of inflammatory mediators (PGE2, PGI2) at the site of application, leading to decreased pain, swelling, and fever. It may also inhibit neutrophil activation and migration.

3. Indications & Uses

  • Localized muscular pain (myalgia)
  • Joint pain (arthralgia) associated with osteoarthritis, rheumatoid arthritis
  • Backache (lumbago)
  • Sprains and strains

4. Dosage & Administration

Adult Dosage: Apply a thin layer to the affected area 3 to 4 times daily. Gently rub in. The amount should be sufficient to cover the area without excessive application. Wash hands after application unless treating hands.

Administration: For external use only. Apply to clean, dry, intact skin. Do not apply on broken skin, open wounds, mucous membranes, eyes, or eczematous areas. Do not use with occlusive dressings unless directed by a physician. Do not apply heat (heating pads) over the area.

5. Side Effects

Common side effects may include:

  • Local skin reactions: Mild burning, stinging, itching, erythema at application site

7. Patient Counselling

  • DO use only on intact skin.
  • DO wash hands before and after application (unless treating hands).
  • DO use the smallest amount needed to cover the area.
  • DO inform your doctor if you are pregnant, planning pregnancy, or breastfeeding.
  • DO inform your doctor about all other medicines you are taking.
  • DON'T apply on cuts, abrasions, infected skin, rashes, or mucous membranes.
  • DON'T use with heating pads, bandages, or occlusive dressings unless advised.
  • DON'T allow the gel to come into contact with eyes, nose, or mouth. If contact occurs, rinse thoroughly with water.
  • DON'T use for more than 7 days for pain or 3 days for fever without consulting a doctor.
  • DON'T use in children under 12 years.

8. Toxicology & Storage

Overdose: Topical overdose (excessive application): Symptoms of salicylism - tinnitus, hearing loss, vertigo, headache, confusion, drowsiness, sweating, hyperventilation (respiratory alkalosis initially, followed by metabolic acidosis), nausea, vomiting, diarrhea. Severe overdose can lead to hyperthermia, hypokalemia, hypoglycemia, seizures, cerebral edema, renal failure, respiratory failure, and cardiovascular collapse.

Storage: Store at room temperature (15-30°C). Protect from light and moisture. Do not freeze. Keep the tube tightly closed. Keep out of reach and sight of children. Do not use after the expiry date printed on the packaging.
