1. Clinical Overview

A topical analgesic and anti-inflammatory combination used for the relief of localized pain and inflammation. Choline salicylate is a non-steroidal anti-inflammatory drug (NSAID) that provides anti-inflammatory and analgesic effects, while lidocaine is a local anesthetic that provides rapid pain relief by blocking nerve signals. This combination is particularly effective for musculoskeletal pain, sore throat, and oral mucosal inflammation.

2. Mechanism of Action

The combination works via two distinct mechanisms. Choline salicylate, after hydrolysis to salicylic acid, inhibits the cyclooxygenase (COX) enzymes, particularly COX-1 and COX-2, leading to reduced synthesis of prostaglandins, which are mediators of pain, inflammation, and fever. Lidocaine acts as a sodium channel blocker on neuronal cell membranes, stabilizing them and preventing the initiation and conduction of nerve impulses, thereby producing local anesthesia.

3. Indications & Uses

  • Topical relief of pain and inflammation in musculoskeletal conditions (e.g., sprains, strains, myalgia, arthritis)
  • Symptomatic relief of sore throat (as a spray or gargle)
  • Pain relief from minor mouth ulcers, stomatitis, and gingivitis

4. Dosage & Administration

Adult Dosage: Apply a thin layer of gel to the affected area 3-4 times daily, or as directed by the physician. For sore throat: Use spray as directed, typically 3-4 sprays every 3-4 hours. Do not exceed 8 applications in 24 hours.

Administration: For external use only. Clean and dry the affected area before application. Apply a thin layer and rub gently. Do not apply on broken skin, open wounds, or mucous membranes unless the formulation is specifically indicated for oral use. Wash hands after application unless treating hands. For oral spray: Direct spray onto affected area in the throat. Do not inhale. Avoid eating or drinking for 15 minutes after application.

5. Side Effects

Common side effects may include:

  • Local burning or stinging sensation at application site
  • Erythema (redness)
  • Skin dryness or peeling
  • Numbness of tongue or mouth (with oral use)

7. Patient Counselling

  • DO use only as directed by your doctor.
  • DO clean and dry the area before applying a thin layer.
  • DO wash your hands after application unless treating hands.
  • DO NOT apply on broken skin, open wounds, or near eyes.
  • DO NOT use with occlusive dressings unless advised.
  • DO NOT use for more than 7 days for pain or 3 days for sore throat without medical advice.
  • DO NOT use in children under 12 years of age.
  • DO NOT use if you are allergic to aspirin or other pain relievers.

8. Toxicology & Storage

Overdose: Topical overdose is rare but possible with excessive application to large areas. Symptoms include: Salicylate Toxicity (Salicylism): Tinnitus, hearing loss, hyperventilation, respiratory alkalosis progressing to metabolic acidosis, nausea, vomiting, confusion, hyperthermia. Lidocaine Toxicity (CNS & Cardiac): Lightheadedness, perioral numbness, metallic taste, tinnitus, visual disturbances, muscle twitching, convulsions, respiratory depression, bradycardia, hypotension, cardiac arrest.

Storage: Store below 30°C. Keep the tube or bottle tightly closed. Protect from light and moisture. Keep out of reach of children. Do not freeze.
