1. Clinical Overview

Cefixime + Clavulanic Acid is a fixed-dose combination of a third-generation cephalosporin antibiotic (Cefixime) and a beta-lactamase inhibitor (Clavulanic Acid). Cefixime acts by inhibiting bacterial cell wall synthesis, while Clavulanic Acid extends its spectrum by irreversibly inhibiting a wide range of beta-lactamase enzymes produced by resistant bacteria, particularly those inactivating cephalosporins. This combination is specifically designed to overcome beta-lactamase-mediated resistance, making it effective against a broader range of pathogens, including many resistant strains common in the Indian clinical setting.

2. Mechanism of Action

Cefixime binds to specific penicillin-binding proteins (PBPs) located inside the bacterial cell wall. This binding inhibits the final transpeptidation step of peptidoglycan synthesis in the bacterial cell wall, leading to the formation of defective cell walls and osmotically unstable cells, resulting in cell lysis and death (bactericidal action). Clavulanic Acid has weak intrinsic antibacterial activity but possesses a high affinity for and irreversibly binds to a wide variety of beta-lactamase enzymes (e.g., TEM-1, SHV-1, and some plasmid-mediated enzymes). This binding 'suicidally' inactivates the enzymes, protecting Cefixime from hydrolysis and extending its spectrum to include beta-lactamase-producing strains.

3. Indications & Uses

  • Acute bacterial exacerbations of chronic bronchitis
  • Community-acquired pneumonia (mild to moderate)
  • Acute uncomplicated cystitis and pyelonephritis
  • Acute otitis media (caused by beta-lactamase producing strains)
  • Pharyngitis and tonsillitis
  • Uncomplicated gonorrhea (urethral and cervical)

4. Dosage & Administration

Adult Dosage: One tablet (Cefixime 200mg + Clavulanic Acid 125mg) every 12 hours. For severe infections or less susceptible organisms, the dose can be increased to one tablet every 8 hours, as per physician discretion.

Administration: Can be taken with or without food. Taking with food may minimize potential gastrointestinal upset. The tablet should be swallowed whole with a full glass of water. Do not crush or chew. Complete the full prescribed course even if you feel better.

5. Side Effects

Common side effects may include:

  • Diarrhea or loose stools
  • Nausea
  • Abdominal pain/discomfort
  • Headache
  • Vaginal candidiasis or itching

7. Patient Counselling

  • DO complete the entire course of medication as prescribed, even if you feel better.
  • DO take the medicine at evenly spaced intervals for best effect.
  • DO inform your doctor if you have a history of kidney disease, liver disease, or allergies.
  • DO report severe diarrhea, abdominal cramps, or blood/mucus in stool immediately.
  • DONT skip doses or double up on a missed dose. Take the next dose at the regular time.
  • DONT share this medication with others, even if they have similar symptoms.
  • DONT take antacids within 2 hours of taking this medicine.

8. Toxicology & Storage

Overdose: Symptoms of overdose are primarily extensions of side effects: severe nausea, vomiting, epigastric distress, diarrhea, and potentially convulsions (especially in renal impairment). Hemodialysis can remove both cefixime and clavulanic acid, reducing serum concentrations.

Storage: Store below 30°C, in a cool, dry place, protected from light and moisture. Keep out of reach of children. Do not use after the expiry date printed on the pack.
