1. Clinical Overview

Carboxymethylcellulose (CMC) 0.5% w/v is a sterile, topical ophthalmic solution classified as an ocular lubricant and demulcent. It is a purified, high molecular weight cellulose derivative that acts as a viscosity-enhancing agent and artificial tear substitute. In the Indian context, it is a first-line, over-the-counter (OTC) therapy for managing dry eye disease (DED), a highly prevalent condition exacerbated by pollution, digital screen use, and climatic conditions. It works by mimicking the mucin layer of the tear film, stabilizing it, and providing prolonged lubrication and protection to the ocular surface.

2. Mechanism of Action

Carboxymethylcellulose is a mucin-mimetic polymer that binds to the hydrophobic ocular surface (cornea and conjunctiva), creating a hydrophilic layer. This layer increases the water-binding capacity of the tear film, reduces tear evaporation, and stabilizes the precorneal tear film. Its anionic nature allows it to interact with positively charged residues on damaged epithelial cells, promoting adhesion and retention.

3. Indications & Uses

  • Dry Eye Disease (Keratoconjunctivitis Sicca) - Aqueous-deficient and Evaporative types
  • Symptomatic relief of dry eye due to environmental factors (AC, pollution, digital eye strain)
  • Post-operative ocular surface lubrication following cataract, refractive, or other eye surgeries

4. Dosage & Administration

Adult Dosage: 1 to 2 drops in the affected eye(s) as needed, typically 3 to 4 times daily. Can be used up to every 1-2 hours for severe dryness under medical supervision.

Administration: 1. Wash hands. 2. Tilt head back. 3. Gently pull down lower eyelid to form a pouch. 4. Instill drop(s) into the pouch without touching the tip to eye, eyelid, or any surface. 5. Close eyes gently for 1-2 minutes. Apply gentle pressure to the nasolacrimal duct (inner corner of eye) for 1 minute to minimize systemic drainage. 6. Wait at least 5 minutes before instilling any other ophthalmic medication.

5. Side Effects

Common side effects may include:

  • Transient blurred vision immediately after instillation
  • Mild ocular discomfort or stinging upon application
  • Sticky eyelashes upon drying of the solution

7. Patient Counselling

  • DO shake the bottle well if instructed on label (for some suspensions).
  • DO use regularly as needed for symptom relief.
  • DO remove contact lenses before application and wait 15 minutes before reinsertion (unless formula is specifically labelled for use with lenses).
  • DONT touch the dropper tip to any surface to avoid contamination.
  • DONT share your eye drops with others.
  • DONT use if the solution changes color or becomes cloudy.

8. Toxicology & Storage

Overdose: Accidental ingestion of bottle contents: Low toxicity expected due to lack of absorption. May cause gastrointestinal discomfort, nausea, or diarrhea. Ocular overdose (excessive drops): Will lead to excessive tearing, blurred vision, and sticky sensation.

Storage: Store at room temperature (15°C to 30°C). Do not freeze. Keep the bottle tightly closed when not in use. Protect from direct light and excessive heat. For unit-dose vials: discard the single-use vial immediately after use, even if solution remains. For multi-dose bottles: discard 28 days after opening. Write the date of opening on the bottle.
