1. Clinical Overview
A multi-ingredient ophthalmic solution combining decongestants (Naphazoline, Phenylephrine), a lubricant (Hydroxypropylmethylcellulose), and cooling agents (Camphor, Menthol). It is primarily indicated for the symptomatic relief of redness, irritation, and discomfort associated with minor eye conditions like allergic conjunctivitis, minor eye irritations, and exposure to environmental factors like dust, smoke, or chlorinated water. The combination provides rapid vasoconstriction, soothing sensation, and lubrication.
| Onset | Duration | Bioavailability |
|---|---|---|
| Naphazoline/Phenylephrine: 2-5 minutes for vasoconstriction. Lubrication: Immediate. | Vasoconstriction: 2-4 hours. Lubrication: Variable, typically 1-2 hours. | Topical ophthalmic; minimal systemic absorption. Estimated <5%. |
2. Mechanism of Action
Naphazoline and Phenylephrine are direct-acting sympathomimetic amines. They act on alpha-1 adrenergic receptors on the conjunctival vasculature, causing vasoconstriction. This reduces blood flow and edema, leading to whitening of the eye. Hydroxypropylmethylcellulose is a demulcent and lubricant that forms a protective film over the ocular surface, relieving dryness and irritation. Camphor and Menthol provide a cooling, soothing sensation via activation of transient receptor potential melastatin 8 (TRPM8) cold receptors, offering subjective relief from discomfort.
3. Indications & Uses
- Symptomatic relief of redness and irritation in allergic conjunctivitis
- Relief of minor eye irritation due to smoke, dust, chlorinated water, or swimming
4. Dosage & Administration
Adult Dosage: Instill 1-2 drops into the affected eye(s) up to 4 times daily, as needed. Do not exceed 4 doses in 24 hours.
Administration: 1. Wash hands before use. 2. Tilt head back. 3. Gently pull lower eyelid down to form a pouch. 4. Instill drops without touching the dropper tip to eye, eyelid, or any surface. 5. Close eyes gently for 1-2 minutes. Apply gentle pressure to the inner corner of the eye (nasolacrimal occlusion) for 1 minute to minimize systemic absorption. 6. Wait at least 5 minutes before instilling any other eye medication. Do not use while wearing soft contact lenses; remove lenses before instillation and wait at least 15 minutes before reinsertion.
5. Side Effects
Common side effects may include:
- Transient stinging/burning on instillation
- Mild blurred vision temporarily
- Pupillary dilation (mydriasis)
- Difficulty in focusing (cycloplegia)
6. Drug Interactions
| Drug | Effect | Severity |
|---|---|---|
| Monoamine Oxidase Inhibitors (MAOIs) - e.g., Phenelzine, Tranylcypromine | Risk of severe hypertensive crisis due to potentiation of sympathomimetic effects. | Contraindicated |
| Tricyclic Antidepressants (TCAs) - e.g., Amitriptyline | Potentiation of pressor effects, increased risk of hypertension and arrhythmias. | Major |
| Beta-blockers (e.g., Timolol eye drops, systemic propranolol) | Paradoxical hypertension and bradycardia; unopposed alpha stimulation. | Moderate |
| Other Sympathomimetics (e.g., systemic decongestants like pseudoephedrine) | Additive adrenergic effects, increasing risk of hypertension, tachycardia, and CNS stimulation. | Moderate |
| Digoxin | Increased risk of cardiac arrhythmias. | Moderate |
| Atropine or other anticholinergics | Additive mydriatic and cycloplegic effects. | Moderate |
7. Patient Counselling
- DO wash hands before use.
- DO use exactly as directed; do not exceed recommended dose or duration (max 3-4 days).
- DO practice nasolacrimal occlusion (pressing inner eye corner) for 1 minute after instillation.
- DO wait at least 5 minutes before using other eye drops.
- DON'T let the dropper tip touch any surface to avoid contamination.
- DON'T use if solution is discolored or contains particles.
- DON'T wear soft contact lenses during treatment.
- DON'T use if you have narrow-angle glaucoma.
8. Toxicology & Storage
Overdose: Ocular: Severe pain, corneal damage, profound mydriasis. Systemic (from ingestion or excessive topical use): Hypertension, tachycardia, arrhythmias, headache, sweating, tremors, CNS effects (restlessness, insomnia, psychosis, or depression), nausea, vomiting. In children: Camphor toxicity can cause nausea, vomiting, lethargy, ataxia, seizures, coma.
Storage: Store at room temperature (15-25°C), protected from light and moisture. Do not freeze. Keep the bottle tightly closed when not in use. Keep out of reach and sight of children. Discard the bottle 28 days after first opening to prevent contamination.
