1. Clinical Overview

A topical combination product containing a potent corticosteroid (Betamethasone) and an astringent, mild antiseptic (Zinc Sulfate). Betamethasone provides potent anti-inflammatory, anti-pruritic, and vasoconstrictive effects, while Zinc Sulfate offers astringent, soothing, and mild antiseptic properties, aiding in wound healing and reducing exudation. This combination is particularly effective for inflammatory dermatoses with exudation or secondary infection risk.

2. Mechanism of Action

Betamethasone binds to cytoplasmic glucocorticoid receptors, forming a complex that translocates to the nucleus. It modulates gene transcription, leading to the synthesis of anti-inflammatory proteins (lipocortin) and inhibition of pro-inflammatory mediators (cytokines, chemokines, prostaglandins, leukotrienes). It also causes vasoconstriction, reducing erythema and edema. Zinc Sulfate acts as an astringent, precipitating proteins to form a protective layer, reducing exudation, and providing a mild antiseptic effect against bacteria like *Staphylococcus aureus*.

3. Indications & Uses

  • Eczematous Dermatitis (Atopic, Seborrheic, Nummular)
  • Contact Dermatitis (Allergic or Irritant)
  • Psoriasis (excluding widespread plaque psoriasis)
  • Lichen Simplex Chronicus
  • Neurodermatitis

4. Dosage & Administration

Adult Dosage: Apply a thin film to the affected area 1-2 times daily. The frequency should be reduced to once daily or on alternate days as the condition improves. Total duration should not exceed 2-4 weeks continuously without medical review.

Administration: Wash and dry the affected area gently. Apply a thin layer and rub in lightly until it disappears. Do not bandage or cover unless directed by a physician. Wash hands after application unless hands are the treatment area. Avoid contact with eyes, mouth, nose, and mucous membranes.

5. Side Effects

Common side effects may include:

  • Local burning or stinging sensation
  • Skin dryness
  • Itching (pruritus) at application site
  • Skin irritation
  • Folliculitis

7. Patient Counselling

  • DO use exactly as prescribed by your doctor.
  • DO apply a thin layer only to the affected area.
  • DO wash your hands before and after application (unless hands are being treated).
  • DO report any signs of infection (increased redness, pus, pain) or lack of improvement.
  • DON'T use on the face, groin, or armpits unless specifically instructed.
  • DON'T cover or bandage the area unless told to do so (occlusion increases absorption and side effects).
  • DON'T use for longer than the prescribed duration.
  • DON'T share this medication with others.

8. Toxicology & Storage

Overdose: Topical: Signs of excessive corticosteroid exposure including severe skin atrophy, striae, telangiectasia, HPA axis suppression (fatigue, weakness, nausea, hypotension, hypoglycemia). Acute oral ingestion of large amounts of cream/ointment is unlikely but may cause GI upset.

Storage: Store at room temperature (15-25°C), protected from light and moisture. Do not freeze. Keep the tube tightly closed. Keep out of reach of children. Do not use after the expiry date printed on the packaging.
