1. Clinical Overview

A fixed-dose combination topical preparation containing a corticosteroid (Beclometasone dipropionate), an antifungal (Clotrimazole), and an aminoglycoside antibiotic (Gentamicin sulfate). It is primarily indicated for the treatment of mixed dermatological infections where inflammation, fungal infection, and bacterial infection coexist, commonly seen in tropical climates like India. It provides anti-inflammatory, antifungal, and antibacterial actions in a single application.

2. Mechanism of Action

The combination works via three distinct mechanisms: 1) Beclometasone suppresses inflammation by inhibiting phospholipase A2, reducing prostaglandin and leukotriene synthesis, and stabilizing lysosomal membranes. 2) Clotrimazole disrupts fungal cell membrane integrity by inhibiting the synthesis of ergosterol, a key component. 3) Gentamicin is bactericidal, inhibiting bacterial protein synthesis by binding irreversibly to the 30S ribosomal subunit.

3. Indications & Uses

  • Infected eczema (bacterial and/or fungal)
  • Infected seborrheic dermatitis
  • Intertrigo with secondary bacterial and/or candidal infection
  • Tinea cruris/corporis with secondary infection
  • Dermatitis with superimposed infection

4. Dosage & Administration

Adult Dosage: Apply a thin layer to the affected area 2 to 3 times daily, or as directed by the physician. Duration should not exceed 2-4 weeks continuously.

Administration: 1. Wash and dry the affected area gently. 2. Take a small amount on clean fingertips. 3. Apply a thin film and rub in gently until it disappears. 4. Wash hands after application unless hands are the treatment site. 5. Do not cover with occlusive dressings unless specifically advised. 6. Do not use for longer than prescribed.

5. Side Effects

Common side effects may include:

  • Local burning or stinging sensation
  • Itching (pruritus)
  • Dryness of skin
  • Erythema (redness)
  • Skin irritation

7. Patient Counselling

  • DO use exactly as prescribed by your doctor.
  • DO wash and dry the area before application.
  • DO use a thin layer; more is not better.
  • DO wash your hands after applying (unless treating hands).
  • DON'T use it on your face, groin, or armpits unless directed.
  • DON'T cover the area with tight bandages or plastic wraps unless told.
  • DON'T use it for longer than the prescribed period (usually 2-4 weeks).
  • DON'T share this medicine with others.

8. Toxicology & Storage

Overdose: Topical: Severe local skin reactions, increased risk of systemic side effects (as listed). Accidental Ingestion: Nausea, vomiting, GI distress. Systemic Gentamicin toxicity: Vertigo, tinnitus, hearing loss, renal impairment.

Storage: Store at room temperature (15-25°C), protected from light and moisture. Do not freeze. Keep the tube tightly closed. Keep out of reach of children. Do not use after the expiry date printed on the pack.
