1. Clinical Overview
Bacterial Lysate (50mg) is an immunomodulator composed of standardized lysates of inactivated bacteria commonly implicated in respiratory tract infections. It functions as a polyvalent bacterial antigenic stimulus, primarily used for the prevention of recurrent respiratory infections by enhancing both innate and adaptive immune responses. It is a non-specific immunostimulant that promotes mucosal and systemic immunity.
| Onset | Duration | Bioavailability |
|---|---|---|
| Immunological effects begin within 1-2 weeks of initiation, but full prophylactic benefit is typically observed after 2-3 months of therapy. | The immunomodulatory effect can persist for several months after completing a standard treatment course, providing protection during high-risk seasons. | Not applicable in conventional pharmacokinetic terms. As an orally administered antigenic preparation, its activity is local (gut-associated lymphoid tissue) and systemic via immune activation, not systemic absorption of active molecules. |
2. Mechanism of Action
Following oral administration, the lysed bacterial antigens interact with the immune system via the gut-associated lymphoid tissue (GALT). They are taken up by M-cells and antigen-presenting cells (APCs) in Peyer's patches. This stimulates a polyclonal B-cell response and the production of specific secretory IgA antibodies, which are transported to mucosal surfaces, particularly the respiratory tract. It also stimulates T-helper cells (Th1 response), enhancing macrophage activity and promoting a balanced immune response.
3. Indications & Uses
- Prophylaxis of recurrent upper and lower respiratory tract infections (e.g., rhinosinusitis, pharyngitis, bronchitis)
- Prevention of acute exacerbations in patients with chronic bronchitis or COPD
- Reduction in frequency and severity of recurrent otitis media in children
4. Dosage & Administration
Adult Dosage: One 50mg capsule orally, once daily on an empty stomach (preferably morning). Standard course: 10 days per month for 3 consecutive months. Alternative: 1 capsule daily for 30 days, followed by a break.
Administration: Swallow the capsule whole with a glass of water, on an empty stomach at least 30 minutes before a meal. Do not crush or chew. For patients unable to swallow capsules, the content can be opened and mixed with a small amount of cold, non-acidic food (e.g., yogurt, apple sauce) immediately before consumption.
5. Side Effects
Common side effects may include:
- Mild gastrointestinal disturbances (nausea, abdominal discomfort, diarrhea)
- Headache
- Skin rash or pruritus
6. Drug Interactions
| Drug | Effect | Severity |
|---|---|---|
| Immunosuppressants (Cyclosporine, Tacrolimus, Azathioprine, high-dose Corticosteroids) | May diminish the immunostimulatory effect of the bacterial lysate. | Moderate |
| Live Attenuated Vaccines | Theoretical risk of excessive immune response. It is advisable to separate administration by at least 2-4 weeks. | Low |
| Other Immunomodulators (e.g., Levamisole, Thymosin) | Potential for additive effect; use with caution under medical supervision. | Moderate |
7. Patient Counselling
- DO take the capsule on an empty stomach as directed.
- DO complete the full 3-month course for optimal prophylactic effect.
- DO inform your doctor if you are planning vaccination.
- DONT take during an active febrile infection; consult your doctor.
- DONT crush or chew the capsule unless advised for mixing with food.
8. Toxicology & Storage
Overdose: No reports of acute overdose. Potential symptoms may include intensified GI side effects (nausea, diarrhea) or flu-like symptoms (fever, malaise) due to excessive immune stimulation.
Storage: Store below 25°C, in a cool, dry place. Protect from light and moisture. Keep out of reach of children. Do not freeze. The strip/blister pack should be kept sealed until use.
