1. Clinical Overview

A fixed-dose combination (FDC) expectorant, antitussive, and antihistamine syrup used for the symptomatic relief of productive and non-productive cough associated with upper respiratory tract infections, allergic conditions, and the common cold. It addresses multiple cough pathways: loosens mucus, suppresses the cough reflex, and reduces allergic/histaminic components.

2. Mechanism of Action

This combination acts synergistically. Ammonium Chloride acts as an expectorant by irritating the gastric mucosa, leading to reflex stimulation of bronchial secretions, thinning mucus. Guaifenesin enhances the output of respiratory tract fluid by reducing the adhesiveness and surface tension of secretions, facilitating easier expectoration. Dextromethorphan is a centrally-acting antitussive that suppresses the cough reflex by elevating the threshold for coughing in the medullary cough center. Chlorpheniramine is a first-generation H1-antihistamine that competitively antagonizes histamine at the H1 receptor, reducing allergic symptoms like rhinorrhea and sneezing, and also possesses mild anticholinergic effects which may contribute to drying of secretions.

3. Indications & Uses

  • Symptomatic relief of dry and productive cough associated with common cold
  • Cough associated with allergic rhinitis
  • Cough with thick, tenacious mucus in upper respiratory tract infections (e.g., bronchitis, tracheitis)

4. Dosage & Administration

Adult Dosage: 10 ml (2 teaspoonfuls) three to four times daily. Maximum: 40 ml per day.

Administration: Shake the bottle well before use. Use the measuring cup or spoon provided. Can be taken with or without food. Taking with a full glass of water may enhance the expectorant effect of Guaifenesin and Ammonium Chloride.

5. Side Effects

Common side effects may include:

  • Dry mouth, nose, or throat
  • Gastrointestinal upset (nausea, vomiting)
  • Dizziness
  • Headache
  • Thickening of bronchial secretions (paradoxical)

7. Patient Counselling

  • DO take the exact dose as prescribed by your doctor.

  • DO inform your doctor about all other medicines you are taking.
  • DO NOT exceed the recommended dose or duration.
  • DO NOT take with alcohol or other sedative medicines.
  • DO NOT drive or operate machinery until you know how it affects you.
  • DO NOT give to children under 2 years of age.
  • DO NOT use for persistent chronic cough (e.g., due to smoking, asthma) without doctor's advice.

8. Toxicology & Storage

Overdose: Symptoms are primarily an extension of side effects: Severe CNS depression (somnolence progressing to stupor/coma) or paradoxical CNS stimulation (hallucinations, agitation, seizures), anticholinergic toxidrome (flushed skin, hyperthermia, dilated pupils, tachycardia, urinary retention, ileus), respiratory depression, nausea/vomiting, ataxia, hyperexcitability (in children). Metabolic acidosis from Ammonium Chloride in large ingestions.

Storage: Store at room temperature (15-30°C), protected from light and moisture. Keep the bottle tightly closed. Keep out of reach and sight of children. Do not use after the expiry date printed on the label. Do not freeze.
