⚠️ Prescription Only: This medicine is Schedule H/H1. Do not self-medicate.

1. Clinical Overview

A fixed-dose combination syrup used for the symptomatic management of productive cough and bronchospasm associated with acute and chronic respiratory conditions. It combines a mucolytic (Ambroxol), a selective beta-2 agonist bronchodilator (Levosalbutamol), and an expectorant (Guaifenesin) to provide comprehensive relief by loosening thick mucus, facilitating its expulsion, and opening the airways.

OnsetDurationBioavailability
Levosalbutamol: 5-15 minutes (inhalation), ~30 minutes (oral). Ambroxol & Guaifenesin: ~30-60 minutes.Levosalbutamol: 6-8 hours. Ambroxol & Guaifenesin: Up to 12 hours.Ambroxol: ~70-80%. Levosalbutamol: ~85-100% (oral). Guaifenesin: Variable, ~45-60%.

2. Mechanism of Action

The combination works synergistically: Ambroxol reduces mucus viscosity by breaking down acid mucopolysaccharide fibers and stimulates surfactant production. Guaifenesin increases respiratory tract fluid secretion, reducing mucus adhesiveness and surface tension. Levosalbutamol relaxes bronchial smooth muscle via beta-2 adrenergic receptor stimulation, relieving bronchospasm and improving mucociliary clearance.

3. Indications & Uses

  • Symptomatic treatment of acute and chronic bronchopulmonary disorders associated with viscid mucus and bronchospasm (e.g., Acute Bronchitis)
  • Chronic Obstructive Pulmonary Disease (COPD) exacerbations with productive cough
  • Bronchial Asthma with difficult expectoration

4. Dosage & Administration

Adult Dosage: 10 ml (2 teaspoonfuls) two to three times daily. Maximum: 30 ml per day.

Administration: Take orally after meals with a full glass of water to minimize gastric upset and aid expectoration. Use the measuring cup/spoon provided. Shake the bottle well before use.

5. Side Effects

Common side effects may include:

  • Nausea
  • Vomiting
  • Diarrhea
  • Gastric irritation/epigastric pain
  • Headache
  • Mild tremor (due to Levosalbutamol)
  • Palpitations
  • Nervousness

6. Drug Interactions

DrugEffectSeverity
Beta-blockers (e.g., Propranolol)Antagonizes bronchodilator effect of Levosalbutamol; may cause severe bronchospasm in asthmatics.Major
Diuretics (e.g., Furosemide, Hydrochlorothiazide)Concurrent use with Levosalbutamol may potentiate hypokalemia.Major
DigoxinHypokalemia from Levosalbutamol may increase risk of digoxin toxicity.Moderate
Monoamine Oxidase Inhibitors (MAOIs) & Tricyclic Antidepressants (TCAs)May potentiate the cardiovascular effects of Levosalbutamol.Moderate
Other Sympathomimetics (e.g., Decongestants)Additive cardiovascular effects (tachycardia, hypertension).Moderate
Xanthine derivatives (Theophylline)Increased risk of CNS stimulation and cardiac effects.Moderate
Anticholinergics (e.g., Ipratropium)Potential additive bronchodilatory effect; also increased risk of urinary retention.Moderate
CNS Depressants (Alcohol, Benzodiazepines)Guaifenesin in high doses may have mild sedative effect; additive sedation possible.Minor

7. Patient Counselling

  • DO take the exact dose prescribed, using the provided measuring device.
  • DO take after meals with plenty of water.
  • DO inform your doctor if you have heart, thyroid, liver, kidney disease, or diabetes.

  • DON'T use if you are pregnant or breastfeeding without medical advice.

8. Toxicology & Storage

Overdose: Symptoms primarily driven by Levosalbutamol: Severe tachycardia, arrhythmias, hypertension, tremor, nervousness, metabolic acidosis, hypokalemia, hyperglycemia. From Ambroxol/Guaifenesin: Severe nausea, vomiting, drowsiness.

Storage: Store at room temperature (15-30°C). Protect from light and moisture. Keep the bottle tightly closed. Keep out of reach of children. Do not use after the expiry date printed on the label.
