1. Clinical Overview

A fixed-dose combination (FDC) medication designed for the symptomatic relief of common cold, allergic rhinitis, and associated conditions. It combines a mucolytic (Ambroxol), a second-generation antihistamine (Levocetirizine), an analgesic-antipyretic (Paracetamol), and a nasal decongestant (Phenylephrine). This combination addresses multiple symptoms simultaneously: cough with thick mucus, nasal congestion, sneezing, runny nose, headache, body ache, and fever. It is widely prescribed in India for upper respiratory tract infections and allergic conditions.

2. Mechanism of Action

The combination exerts a multi-targeted symptomatic effect. Ambroxol stimulates surfactant production and serous secretion in the respiratory tract, breaking down acid mucopolysaccharide fibers in sputum. Levocetirizine is a potent and selective peripheral H1-receptor antagonist, inhibiting histamine-mediated allergic symptoms. Paracetamol acts centrally by inhibiting cyclooxygenase (COX) enzymes, particularly in the CNS, reducing prostaglandin synthesis involved in pain and fever. Phenylephrine is a selective alpha-1 adrenergic receptor agonist causing vasoconstriction in the nasal mucosa, reducing edema and congestion.

3. Indications & Uses

  • Symptomatic relief of Common Cold
  • Symptomatic relief of Allergic Rhinitis (Seasonal/Perennial)
  • Acute Sinusitis with associated symptoms
  • Acute Nasopharyngitis (with thick mucus and congestion)

4. Dosage & Administration

Adult Dosage: One tablet twice daily. Maximum: 2 tablets in 24 hours. Should be taken after food with a full glass of water.

Administration: Oral administration. Swallow whole with water, do not crush or chew. Best taken after meals to minimize gastric irritation. Maintain adequate hydration to aid ambroxol's mucolytic action. Do not take for more than 5 days for fever or 3 days for pain without medical advice.

7. Patient Counselling

  • DO take after food with plenty of water.
  • DO complete the prescribed course but do not self-medicate beyond 3-5 days for fever/pain.
  • DO inform your doctor about all other medicines you are taking.
  • DO monitor for signs of allergic reaction.
  • DONT consume alcohol while on this medication.
  • DONT drive or operate heavy machinery if feeling drowsy.
  • DONT take with other paracetamol-containing products (Crocin, Dolo, etc.).
  • DONT give to children below 12 years.

8. Toxicology & Storage

Overdose: Paracetamol: Nausea, vomiting, anorexia, abdominal pain, pallor, sweating. After 24-72h: Right upper quadrant pain, elevated LFTs, jaundice, hepatic necrosis, hypoglycemia, coma. Levocetirizine: Drowsiness, agitation, restlessness, tachycardia. Ambroxol: GI distress. Phenylephrine: Severe hypertension, headache, palpitations, reflex bradycardia, cardiac arrhythmias, pulmonary edema.

Storage: Store below 30°C, in a cool, dry place. Protect from light and moisture. Keep out of reach of children. Do not use after the expiry date printed on the pack.
