Acetylcysteine (600mg) + Astaxanthin (4mg) + Clomiphene (100mg)

Clinical Pharmacologist's Monograph

⚠️ Prescription Only: This medicine is Schedule H/H1. Do not self-medicate.

1. Clinical Overview

This is a novel, non-standard combination product primarily used in the Indian context for the management of male infertility, specifically oligospermia and asthenospermia, with adjunctive antioxidant support. Acetylcysteine provides mucolytic and antioxidant effects, Astaxanthin is a potent carotenoid antioxidant, and Clomiphene citrate is a selective estrogen receptor modulator (SERM) used to stimulate gonadotropin secretion. This combination is not a first-line therapy per international guidelines but is prescribed based on empirical evidence and clinical experience in India for improving semen parameters.

OnsetDurationBioavailability
Clomiphene: Endocrine effects begin within days, but full spermatogenic effects may take 3-6 months. Acetylcysteine/Astaxanthin: Systemic antioxidant effects are gradual over weeks.Clomiphene: Effects can persist for weeks after discontinuation due to long half-life and hypothalamic-pituitary effects. Acetylcysteine: 5-6 hours. Astaxanthin: Up to 72 hours due to lipid accumulation.Acetylcysteine: ~10% (oral). Astaxanthin: Variable, enhanced with fatty meals. Clomiphene: High but variable.

2. Mechanism of Action

The combination works synergistically: Clomiphene blocks estrogen receptors at the hypothalamus and pituitary, reducing negative feedback and increasing secretion of GnRH, FSH, and LH. Elevated FSH stimulates spermatogenesis in Sertoli cells, while LH stimulates Leydig cells to produce testosterone. Acetylcysteine, a precursor to glutathione, acts as a direct and indirect antioxidant, reducing oxidative stress in seminal plasma which is detrimental to sperm motility and DNA integrity. Astaxanthin, a powerful lipid-soluble antioxidant, protects sperm cell membranes from lipid peroxidation, improving sperm motility and morphology.

3. Indications & Uses

  • Male infertility due to oligospermia (low sperm count)
  • Male infertility due to asthenospermia (poor sperm motility)
  • Idiopathic male infertility with high seminal oxidative stress

4. Dosage & Administration

Adult Dosage: One tablet daily, preferably after a meal (to enhance Astaxanthin absorption). Duration: Typically prescribed for 3-6 months, reassessed based on semen analysis.

Administration: Swallow whole with a full glass of water. Take with food, preferably a meal containing fats (e.g., milk, nuts) to improve Astaxanthin absorption. Avoid taking with antacids within 2 hours as they may interfere with absorption. Do not crush or chew.

5. Side Effects

Common side effects may include:

  • Headache
  • Nausea, abdominal discomfort
  • Hot flushes (due to Clomiphene)
  • Visual disturbances (blurring, spots - Clomiphene)
  • Dizziness
  • Skin rash/itching

6. Drug Interactions

DrugEffectSeverity
Warfarin and other anticoagulantsClomiphene may alter protein binding and increase bleeding risk. Acetylcysteine may have mild anticoagulant effect.Major
Thyroid hormonesClomiphene may increase serum thyroxine-binding globulin, altering thyroid function tests and potentially dose requirements.Moderate
CYP2C9/2C19/3A4 inducers (e.g., Rifampicin, Carbamazepine, Phenytoin)May decrease plasma levels of Clomiphene, reducing efficacy.Moderate
CYP2C9/2C19/3A4 inhibitors (e.g., Fluconazole, Ketoconazole, Clarithromycin)May increase plasma levels of Clomiphene, increasing toxicity risk.Moderate
Nitroglycerin and other vasodilatorsConcurrent use with Acetylcysteine may cause additive hypotensive effects.Moderate
Activated CharcoalMay reduce absorption of Acetylcysteine and Astaxanthin if taken simultaneously.Minor

7. Patient Counselling

  • DO take the tablet once daily after a meal, preferably with fats like milk or nuts.
  • DO attend all follow-up appointments for semen analysis and hormone tests (typically every 3 months).
  • DO inform your doctor about all other medicines, including supplements, you are taking.
  • DO use effective contraception if your female partner is not attempting conception, as the medicine's effects are intended for planned pregnancy.
  • DONT take a double dose if you miss one. Skip it and take the next dose at the regular time.
  • DONT stop the medication abruptly without consulting your doctor.
  • DONT share this medicine with others, especially women, as it can cause serious harm.

8. Toxicology & Storage

Overdose: Clomiphene overdose: Severe nausea, vomiting, vasomotor flushes, visual disturbances, abdominal pain, ovarian enlargement (in females). Acetylcysteine overdose: Nausea, vomiting, diarrhea, urticaria, bronchospasm (rare with oral). Astaxanthin overdose: May cause carotenodermia (orange discoloration of skin). General: Dizziness, confusion.

Storage: Store in a cool, dry place, protected from light and moisture. Keep the container tightly closed. Store at room temperature (15-30°C). Keep out of reach of children and pets. Do not use after the expiry date printed on the pack.
