📋 Clinical Overview

A fixed-dose combination topical preparation containing a corticosteroid (Beclometasone), an antifungal (Clotrimazole), and an aminoglycoside antibiotic (Gentamicin). It is primarily indicated for the treatment of mixed dermatological infections where inflammation, fungal overgrowth, and bacterial infection coexist, commonly seen in conditions like infected eczema, intertrigo, and infected dermatophytosis in the Indian climatic context.

💊 Dosage & Administration

Adult: Apply a thin film to the affected area 2 to 3 times daily, or as directed by the physician. Duration should not exceed 4 weeks continuously.

Note: Wash and dry the affected area thoroughly. Apply a thin layer and rub gently. Wash hands after application unless hands are the treated area. Do not cover with occlusive dressings unless advised. Do not use in or near the eyes.

⚠️ Contraindications

  • Hypersensitivity to Beclometasone, Clotrimazole, Gentamicin, or any component
  • Viral skin infections (herpes simplex, chickenpox)
  • Fungal keratitis (for ophthalmic use)
  • Primary bacterial skin infections without inflammation (use plain antibiotic)
  • Tuberculous or syphilitic skin lesions

🔬 Mechanism of Action

This combination exerts a triple action: Beclometasone provides potent anti-inflammatory, anti-pruritic, and vasoconstrictive effects; Clotrimazole inhibits the synthesis of ergosterol, a critical component of fungal cell membranes, leading to increased permeability and cell death; Gentamicin is bactericidal, inhibiting bacterial protein synthesis by binding to the 30S ribosomal subunit.

🤰 Special Populations

Pregnancy: Category C (US FDA). Topical corticosteroids, clotrimazole, and gentamicin can be absorbed in small amounts. Use only if clearly needed and potential benefit justifies potential risk to the fetus. Avoid large amounts or long-term use.

Driving: No effect expected from topical use.
