📋 Clinical Overview

A fixed-dose combination (FDC) medication primarily indicated for the management of neuropathic pain, particularly diabetic peripheral neuropathy (DPN) and post-herpetic neuralgia (PHN), with adjunctive B-vitamins aimed at addressing potential nutritional deficiencies and supporting nerve health. Pregabalin is a centrally-acting analgesic, while Methylcobalamin, Pyridoxine, and Folic Acid are neurotropic B-vitamins. This combination is widely prescribed in India for conditions involving nerve damage and pain, though the clinical superiority of the FDC over pregabalin monotherapy is debated.

💊 Dosage & Administration

Adult: One capsule/tablet twice daily (Total: Pregabalin 300mg/day, Methylcobalamin 3000mcg/day, etc.). Initiation often starts at a lower dose (e.g., Pregabalin 75mg) and titrated upward based on response and tolerability.

Note: Can be taken with or without food. Swallow whole with a glass of water. Do not crush or chew. For optimal B-vitamin absorption, taking on an empty stomach may be advised, but tolerability with pregabalin may favor taking with food.

⚠️ Contraindications

  • Hypersensitivity to Pregabalin, Methylcobalamin, Pyridoxine, Folic Acid or any excipient.
  • Patients with a history of angioedema with pregabalin therapy.

🔬 Mechanism of Action

Pregabalin binds with high affinity to the alpha2-delta subunit of voltage-gated calcium channels in the central nervous system, reducing calcium influx and subsequent release of excitatory neurotransmitters (e.g., glutamate, substance P, norepinephrine). This dampens neuronal hyperexcitability, leading to analgesic, anxiolytic, and anticonvulsant effects. Methylcobalamin is essential for myelin synthesis and maintenance of neuronal integrity. Pyridoxine (as PLP) is a cofactor in neurotransmitter synthesis (GABA, serotonin, dopamine). Folic Acid (as 5-MTHF) is involved in nucleotide synthesis and homocysteine metabolism; elevated homocysteine is implicated in neuropathy.

🤰 Special Populations

Folic Acid is recommended in pregnancy but at lower doses (400-800 mcg).

Driving: WARN: May cause dizziness and somnolence. Patients should not drive or operate heavy machinery until they know how the medication affects them, especially during initiation and dose escalation.
