📋 Clinical Overview

A fixed-dose combination topical gel/cream primarily indicated for the treatment of mild to moderate acne vulgaris. Clindamycin is a lincosamide antibiotic that reduces the population of Cutibacterium acnes and exhibits anti-inflammatory properties. Tretinoin is a retinoid that normalizes follicular keratinization, reduces comedone formation, and enhances the penetration of clindamycin. This combination targets multiple pathogenic factors of acne: bacterial proliferation, inflammation, and abnormal desquamation.

💊 Dosage & Administration

Adult: Apply a thin film to the affected area(s) once daily, preferably in the evening. Start with alternate-day application if irritation occurs.

Note: 1. Wash face gently with a mild cleanser and pat dry. 2. Wait 20-30 minutes for skin to dry completely. 3. Dispense a pea-sized amount for the entire face. 4. Apply a thin layer, avoiding eyes, lips, nostrils, and mucous membranes. 5. Wash hands after application. Use a non-comedogenic moisturizer and broad-spectrum sunscreen (SPF 30 or higher) during the day.

⚠️ Contraindications

  • Hypersensitivity to clindamycin, lincomycin, tretinoin, or any component of the formulation
  • History of regional enteritis, ulcerative colitis, or antibiotic-associated colitis
  • Concurrent use with erythromycin-containing topical products due to antagonism

🔬 Mechanism of Action

The combination exerts a synergistic, multi-targeted action against acne vulgaris. Clindamycin inhibits bacterial protein synthesis, reducing the population of Cutibacterium acnes and its associated pro-inflammatory mediators. It also directly inhibits leukocyte chemotaxis. Tretinoin binds to retinoic acid receptors (RARs) in the skin, normalizing the differentiation of follicular keratinocytes, preventing microcomedone formation, and promoting the expulsion of existing comedones. It also enhances the penetration of clindamycin into the pilosebaceous unit.

🤰 Special Populations

Pregnancy: Category C (US FDA). Tretinoin is teratogenic in oral form; topical use is generally considered low risk due to minimal absorption, but should be avoided unless clearly needed. Clindamycin topical is Category B. Use only if potential benefit justifies potential risk to fetus. Consultation with an obstetrician is recommended.
