📋 Clinical Overview

Tolnaftate is a synthetic, broad-spectrum, thiocarbamate antifungal agent used topically for the treatment of superficial dermatophytosis (tinea infections) and pityriasis versicolor. It acts by inhibiting fungal squalene epoxidase, leading to ergosterol depletion and fungal cell membrane disruption. It is considered a first-line topical antifungal in India due to its efficacy, safety, and low cost.

💊 Dosage & Administration

Adult: Apply a thin layer to cover the affected and immediately surrounding area twice daily (morning and evening).

Note: Wash and dry the affected area thoroughly before application. Apply with clean hands or applicator. Rub gently until absorbed. Wash hands after application unless hands are being treated. For tinea pedis, pay special attention to spaces between toes. Continue use for at least 2 weeks after symptoms disappear to prevent recurrence (typically 4-6 weeks total).

  • Known hypersensitivity to tolnaftate or any component of the formulation (e.g., butylated hydroxytoluene).

🔬 Mechanism of Action

Tolnaftate is a fungicidal and fungistatic agent. Its primary mechanism is the selective inhibition of the fungal enzyme squalene epoxidase. This enzyme is crucial in the early stages of ergosterol biosynthesis. Inhibition leads to the intracellular accumulation of squalene and a deficiency of ergosterol, an essential component of the fungal cell membrane. This disrupts membrane structure and function, increasing permeability and leading to fungal cell death.

Pregnancy: Category C (US FDA). Animal reproduction studies have not been conducted. Due to negligible systemic absorption, risk to the fetus is considered low. Use only if clearly needed and benefits outweigh risks. Preferred topical antifungal in pregnancy in many Indian guidelines.
