📋 Clinical Overview

A fixed-dose combination (FDC) of a sympathomimetic decongestant (Phenylephrine) and a first-generation alkylamine antihistamine (Triprolidine). It is primarily indicated for the symptomatic relief of allergic rhinitis and the common cold by reducing nasal congestion, sneezing, rhinorrhea, and itchy/watery eyes. Phenylephrine acts as a selective α1-adrenergic agonist, causing vasoconstriction in the nasal mucosa. Triprolidine is a potent H1-receptor antagonist with anticholinergic and sedative properties. This combination is widely used in the Indian OTC market for upper respiratory allergies and colds.

💊 Dosage & Administration

Adult: One tablet every 6 to 8 hours. The maximum recommended frequency is 4 times in 24 hours. Should not be used for more than 7 days for self-medication of cold symptoms.

Note: Administer orally with or without food. Taking with food may reduce potential GI upset. Tablet should be swallowed whole with a full glass of water. Do not crush or chew. Avoid taking dose late in the evening if sedation is undesirable during daytime.

  • Concurrent use or within 14 days of MAO inhibitor therapy (risk of hypertensive crisis)
  • Severe Hypertension
  • Severe Coronary Artery Disease
  • Patients on Beta-blockers (non-selective) - risk of unopposed alpha agonism leading to severe hypertension
  • Narrow-angle Glaucoma
  • Symptomatic Prostatic Hyperplasia with urinary retention
  • Pyloroduodenal obstruction
  • Breastfeeding

🔬 Mechanism of Action

The combination provides symptomatic relief through two distinct mechanisms. Phenylephrine acts as a direct sympathomimetic agonist on post-synaptic α1-adrenergic receptors in the vascular smooth muscle of the nasal mucosa. This causes vasoconstriction, reducing blood flow, edema, and nasal congestion. Triprolidine competitively and reversibly inhibits histamine at the H1 receptor sites, blocking the effects of endogenous histamine released during allergic reactions. It also possesses significant anticholinergic (antimuscarinic) activity, contributing to the drying of secretions.

🤰 Special Populations

Pregnancy: Pregnancy Category C (US FDA). Animal reproduction studies are not available. Use only if clearly needed and potential benefit justifies potential risk to the fetus. Phenylephrine may reduce placental blood flow. Triprolidine has been associated with rare teratogenic effects in some studies. Avoid in first trimester unless absolutely necessary.

Driving: NOT ADVISABLE. Triprolidine causes significant drowsiness and impairs cognitive and motor functions. Patients should not drive or operate heavy machinery until their individual response is known (usually 24-48 hours after initiation).
