Sodium Chloride 0.65% w/w is a sterile, isotonic, buffered, or non-preserved aqueous solution of sodium chloride, primarily used as a nasal irrigation/drop and ophthalmic solution. It is classified as a nasal decongestant, mucolytic, and ocular lubricant. Its primary mechanism is osmotic, drawing fluid out of edematous nasal and ocular tissues to reduce congestion and irritation. It provides mechanical cleansing and humidification of the nasal passages and ocular surface, helping to clear mucus, allergens, and irritants. In the Indian context, it is widely available over-the-counter (OTC) and is a first-line, non-pharmacological intervention for nasal congestion associated with common cold, allergic rhinitis, and sinusitis, as well as for dry eye syndrome.
Adult: Nasal Drops/Spray: 2-3 drops or 1-2 sprays into each nostril, 3-4 times daily or as needed. Nasal Irrigation: Use as directed with a neti pot or irrigation bottle, typically once or twice daily. Ophthalmic: 1-2 drops in each eye, as needed for dryness, up to 4-6 times daily.
Note: Nasal: Tilt head back slightly for drops/spray. For irrigation, use lukewarm, previously boiled, or sterile water if preparing solution from packets. Use proper technique to avoid forcing fluid into Eustachian tubes. Ophthalmic: Wash hands, tilt head back, pull lower eyelid down, instill drops without touching dropper to eye or any surface. Wait 5 minutes before instilling other eye medications. Discard single-use vials immediately after use.
Sodium Chloride 0.65% w/w exerts its effects primarily through osmotic action. When applied to the nasal mucosa or ocular surface, the hypotonic solution creates an osmotic gradient that draws fluid out of edematous tissues (in the nose) and hydrates the epithelial surfaces. In the nose, this reduces mucosal swelling, thins viscous mucus, and facilitates its clearance. It also mechanically flushes out allergens, dust, pollutants, and infectious agents. For the eyes, it acts as a tear substitute, lubricating the ocular surface, relieving dryness, and washing away irritants.
Pregnancy: Category A (US FDA). Sodium chloride is considered safe for use during pregnancy. Topical use for nasal/ocular symptoms is acceptable. However, as with all medications, use should be limited to when clearly needed.
Driving: No effect. Ophthalmic use may cause temporary blurring of vision; patients should wait until vision clears before driving or operating machinery.
